Il s’est construit autour d’un triptyque alliant médecine générale, neurosciences et sciences humaines et sociales. La première partie a étayé la subjectivité du médecin par rapport à la plainte d’insomnie. Ceux qui prenaient des somnifères les prescrivaient plus facilement que leurs confrères. La réponse thérapeutique proposée aux patients reflétait le soin ou la négligence que le médecin portait à son propre sommeil. La deuxième partie a analysé 102 consultations avec plainte d’insomnie, extraites de 1188 consultations observées avec un dispositif méthodologique mixte. Un effet du déroulement temporel de la consultation sur la prescription est apparu, de la plainte apparaissant précocement jusqu’à celle de fin de consultation. Nous avons évalué l’agenda du sommeil, et pu définir à partir des facteurs influençant la qualité du remplissage de ces agendas par les patients, des conditions pour l’utiliser en médecine générale. La luminothérapie remplit des conditions théoriquement intéressantes pour l’insomnie. La méta-analyse a confirmé un effet bénéfique sur certains critères du sommeil, sa mise en application reste à préciser. This thesis explores in a spin-off from general medicine consultation four aspects of insomnia. It is built around a triptych combining general medicine, neurosciences and human and social sciences. The first part highlighted the subjectivity of the physician towards the complaint of insomnia. Those who took sleeping pills were more likely to prescribe them than their colleagues. The therapeutic response offered to patients reflected the physician's care or neglect of one' s own sleep. The second part analyzed 102 consultations with complaint of insomnia, extracted from 1188 consultations observed with mixed methodological design. An effect of the chronological course of the consultation on the prescription emerged, going from the complaint appearing early to the complaint at the end of the consultation. We evaluated the sleep diary, identifying the factors influencing the quality of the patients' filling of these diaries. This enabled us to define the conditions for its use in general medicine. Light therapy fulfills theoretically interesting conditions for insomnia. The meta-analysis confirmed a beneficial effect on certain sleep criteria, its application remains to be specified.
