<dcterms:abstract xml:lang="fr">Les lésions d'ischémie-reperfusion hépatique (IRI) résultent de traumatismes, de transplantations hépatiques ou de chirurgies hépatobiliaires. Aucun outil intraopératoire et non invasif ne prédit efficacement la viabilité du foie. L'évaluation de la circulation hépatique par échographie (US) est entravée par des contraintes de temps, en particulier dans les procédures laparoscopiques, et par la dépendance à l'opérateur. L'US localise et quantifie de manière insuffisante l'oxygénation de la surface hépatique. L'imagerie hyperspectrale (HSI) émerge comme une modalité non invasive pour la quantification de la perfusion tissulaire intraopératoire. En détectant la réflectance lumineuse entre 500 et 1000 nm, la HSI quantifie des composés organiques tels que l'hémoglobine. De manière prometteuse, la HSI permet la quantification intraopératoire de la perfusion hépatique et de l'oxygénation, discernant différents types d'ischémie hépatique. Les préréglages du système HSI de notre étude incluent StO2% (saturation relative en oxygène jusqu'à ~1 mm de profondeur), NIR (oxygénation relative à une profondeur de 3 à 5 mm) et TWI% (distribution d'eau à une profondeur de 3 à 5 mm). La mise en œuvre de l'intelligence artificielle et de la réalité augmentée renforce le potentiel du système. Des tests in vitro ont exploré la quantification de molécules exogènes, prélude à des expériences in vivo. La thèse souligne le potentiel de la HSI pour la prédiction de la viabilité hépatique et l'orientation intraopératoire, en exploitant la réalité augmentée.</dcterms:abstract>
<dcterms:abstract xml:lang="en">Liver ischemia-reperfusion injury (IRI) results from trauma, liver transplantation, or hepatobiliary surgery. No effective intraoperative, non-invasive tool predicts liver viability. Hepatic circulation assessment using ultrasound (US) is hindered by time constraints, especially in laparoscopic procedures, and operator dependence. US inadequately localizes and quantifies oxygenation across the liver surface. Hyperspectral imaging (HSI) emerges as a non-invasive modality for intraoperative tissue perfusion quantification. Detecting light reflectance between 500 and 1000nm, HSI quantifies organic compounds like hemoglobin. Promisingly, HSI enables intraoperative quantification of hepatic perfusion and oxygenation, discerning various liver ischemia types. Our study's HSI system presets include StO2% (relative oxygen saturation up to ~1mm depth), NIR (relative oxygenation at 3 to 5mm depth), and TWI% (water distribution at 3 to 5mm depth). Implementing artificial intelligence and augmented reality enhances the system's potential. In vitro tests explored quantifying exogenous molecules, a prelude to in vivo experiments. The thesis underscores HSI's potential for liver viability prediction and intraoperative guidance, leveraging enhanced reality.</dcterms:abstract>
