<dcterms:abstract xml:lang="fr">L’ouverture de la Barrière Hémato-Encéphalique (oBHE) par Ultrasons Focalisés (UF) est une technique qui pourrait améliorer considérablement les effets thérapeutiques des médicaments dédiés aux maladies du cerveau. La BHE est en effet responsable de l’inefficacité de presque 100 % des molécules visant ces pathologies. La transposition de la technique d’oBHE par UF vers une application clinique est très récente. Plusieurs essais chez l’humain sont actuellement en cours, avec des solutions cliniques différentes. Dans ce travail de thèse, nous étudions une solution où le transducteur ultrasonore est positionné grâce à un manipulateur robotique guidé par neuronavigation. L’intérêt de la robotique dans ce contexte est de pouvoir traiter des zones cibles plus larges par la génération de trajectoires complexes adaptées aux contraintes acoustiques du problème. La méthode proposée permet de réaliser la planification d’un traitement personnalisé pour une anatomie de patient donnée. Le modèle de trajectoire de balayage élaboré permet théoriquement d’ouvrir la BHE sur des zones cibles étendues de l'ordre d'une dizaine de cm².</dcterms:abstract>
<dcterms:abstract xml:lang="en">Blood-Brain Barrier (BBB) opening with Focused Ultrasound (FU) is a technique that could significantly improve drug delivery into the brain, especially for treating tumors and neurodegenerative diseases. As a natural selective frontier, the BBB prevents almost 100 % of injected therapeutic agents to reach the brain cells. About fifteen years of preclinical studies have shown the efficiency and safety of using FU to open the BBB in order to facilitate bioavailability of drugs. Different devices are currently under clinical studies in order to evaluate the safety of this technique on the human model. The topic of this thesis is to introduce a new solution based on the use of a neuronavigation-guided robotic manipulator to perform BBB opening. The main interest of using robotics in this task is the possibility to target large volumes by generating complex trajectories which parameters are deduced from acoustic considerations. A planning method enabling to generate a patient-dedicated treatment was designed and tested. A sweeping trajectory generator was implemented and can theoretically induce BBB opening on a target surface of about ten cm².</dcterms:abstract>
