Le traitement de ces stades repose sur une chirurgie mini-invasive avec l’exérèse de la tumeur primitive et la biopsie du ganglion sentinelle (GS) pelvien, pour obtenir l’ensemble des informations pronostiques afin de définir une prise en charge adaptée. La problématique de cette pathologie réside, d’une part, dans une évaluation radiologique préopératoire imprécise et d’autre part, dans des difficultés de détection du GS en peropératoire dues aux limites des techniques de détection standards. Afin d’optimiser les performances diagnostiques de l’IRM préopératoire, nous avons développé un modèle prédictif d’intelligence artificielle (IA) dont les résultats préliminaires confirment son aide stratégique en oncologie. Pour permettre une identification plus fiable et reproductible du GS, un système de navigation en réalité augmentée basé sur un recalage intrinsèque et sur l’utilisation d’un bras collaboratif robotique a été développé permettant la localisation spatiale du GS en temps réel et en continu. Endometrial cancer is the most common gynecological cancer in developed countries, with 80% of cases diagnosed at an early stage. The treatment of these stages is based on minimally invasive surgery with the removal of the primary tumor and biopsy of the pelvic sentinel lymph node (SLN), to obtain all the prognostic information needed to define appropriate management. The challenges to be overcome are, on the one hand, an imprecise preoperative radiological evaluation and, on the other hand, the difficulties of detection of the SLN intraoperatively due to the limits of standard detection techniques. In order to optimize the diagnostic performance of preoperative MRI, we have developed an artificial intelligence (AI) predictive model whose preliminary results confirm its strategic use in oncology. To allow more reliable and reproducible identification of the SLN an augmented reality navigation system based on intrinsic registration and the use of a robotic collaborative arm has been developed allowing the spatial localization of the SLN continuously and in real time.
