<dcterms:abstract xml:lang="fr">Introduction Douze mille laryngectomies totales sont réalisées en Europe chaque année pour des cancers laryngés avancés. Matériel et Méthode Nous avons évalué la possibilité d’utiliser une allogreffe aortique en remplacement laryngé pour franchir une nouvelle étape dans l’histoire du Larynx Artificiel, initialement conçu par notre équipe en titane, de façon à se rapprocher au mieux de la souplesse des tissus initiaux. Résultats Des études in vitro ont permis de mettre en évidence que l’aorte pouvait être une bonne matrice en termes de transformation cartilagineuse et de ré-épithélialisation de surface. Des études in vivo sur brebis et rat ont permis de confirmer la possibilité d’une bonne intégration de l’allogreffe aortique en remplacement laryngo-trachéal. Conclusion Ces études d’ingénierie laryngo-trachéale ont permis d’aboutir à la première application clinique d’allogreffe aortique au niveau laryngé.</dcterms:abstract>
<dcterms:abstract xml:lang="en">Introduction Twelve thousand total laryngectomies are performed in Europe each year for advanced laryngeal cancers. Material and Method We evaluated the possibility of using an aortic allograft in laryngeal replacement to take a new step in the History of the Artificial Larynx, initially designed by our team in titanium, in order to best approach the flexibility of the initial’s tissues. Results In vitro studies demonstrated that the aorta could be a good matrix in terms of cartilage transformation and surface re-epithelialization. In vivo studies on sheep and rats confirmed the possibility of good integration of the aortic allograft in laryngotracheal replacement. Conclusion These laryngo-tracheal engineering studies led to the first clinical application of aortic allograft in laryngeal replacement.</dcterms:abstract>
