<dcterms:abstract xml:lang="fr">Les progrès en imagerie médicale, notamment la tomodensitométrie, ont considérablement amélioré les soins aux patients en permettant la visualisation in vivo des structures anatomiques en 3D. C'est dans ce contexte que s'inscrit la société Visible Patient, spécialisée dans la création de modèles 3D à partir de segmentations réalisées par des experts en imagerie médicale. Néanmoins, la segmentation manuelle de ces structures est chronophage et sujette à variations. Pour pallier cela, nous introduisons une méthode d'apprentissage profond pour la segmentation des structures thoraciques. L'architecture proposée en cascade comprend deux étapes : la segmentation des poumons, puis celle des structures internes en utilisant des scanners masqués par les poumons. Chaque étape utilise une approche de traitement tri-directionnelle afin d'exploiter les informations axiales, coronales et sagittales par un processus de fusion. Plusieurs configurations de fusion sont explorées dans le but d'obtenir des masques de segmentation finaux optimaux. La performance de la méthode est évaluée à travers des expériences sur un échantillon de 196 patients, comprenant la robustesse aux pathologies et les scanners sans agents de contraste. L'approche proposée obtient des performances similaires à la procédure standard de Visible Patient tout en divisant par près de 3,5 le temps requis, ce qui se traduit par une économie de jusqu'à trois heures en moyenne.</dcterms:abstract>
<dcterms:abstract xml:lang="en">Advances in medical imaging, specifically CT scans, have significantly improved patient care by allowing in vivo visualization of anatomical structures in 3D. It is in this context that the company Visible Patient operates, specializing in the creation of 3D models from segmentations conducted by experts in medical imaging. However, manual segmentation of these structures is time-consuming and subject to variations. To address this, a deep learning method is introduced in this work for segmenting thoracic structures. The proposed cascaded architecture includes two steps: lung segmentation and segmentation of internal structures using lung-masked CT scans. Each step utilizes a three-path processing approach to leverage axial, coronal, and sagittal information through a fusion process. Multiple fusion configurations are explored trying to obtain optimal final segmentation masks. The method's performance is evaluated through experiments on a dataset of 196 patients, including pathology robustness and CT scans without contrast agents. The proposed approach achieves similar performance to the Visible Patient standard procedure while dividing the required time by almost 3.5, resulting in savings of up to three hours on average.</dcterms:abstract>
