Freins à l'adressage en psychothérapie des patients déprimés par les médecins généralistes d'Alsace

Description : Médecine générale, Introduction : la dépression est une pathologie centrale de la médecine générale. Selon les recommandations de bonne pratiques, le traitement de première intention des formes légère à modérée est la psychothérapie. La densité des professionnels de la santé mentale en France est parmi les plus hautes d’Europe. Pourtant les généralistes français adressent leurs patients dépressifs pour psychothérapie à des taux particulièrement bas. Quels sont les freins à l'adressage en psychothérapie des patients déprimés par les médecins généralistes, en Alsace ? Matériel et méthodes : une étude transversale d'épidémiologie descriptive a été réalisée de mai à août 2019, auprès de l'ensemble des médecins généralistes d'Alsace. Les participants ont été invités, par mail, à répondre à un questionnaire auto administré, construit à partir d'une revue de la littérature. Résultats : nous avons retrouvé 3 freins principaux évoqués par les généralistes dans le processus d'adressage en psychothérapie : le coût des thérapies (75%), la difficulté d'accès (65%), et la réticence des patients (62%). Des facteurs influençant le processus d'adressage ont été retrouvés. L'aisance des généralistes dans la prise en charge de la dépression, le fait d'avoir une opinion positive quant à l'efficacité de la psychothérapie, ainsi que d'en avoir bénéficié à titre personnel, étaient reliés de manière statistiquement significative à un adressage plus élevé. Conclusion - perspectives : l'adressage en psychothérapie est un processus complexe. Plusieurs pistes d'améliorations sont possibles : veiller à la qualité de la formation universitaire des généralistes, afin de favoriser une appropriation de l'enseignement. Du côté des patients, il s'agit de poursuivre la dé stigmatisation, un des objectifs principal de l'OMS depuis 1998. Quant au système de soins secondaire, l'Assurance Maladie, inspirée par le projet britannique IAPT (Impoving Acces to Psychological Therapies) est en cour d'expérimentation (2018-2020), permettant le remboursement de séances de psychothérapie dans 3 départements., Introduction : Depression is a core disease in general medicine. According to the official recommandations, light to moderate depression is to be treated by psychotherapy first and foremost. However few patients are treated by psychotherapy alone. In France, the density of mental health professional is one of the highest in Europe, while the patient's referring rate, one of the lowest. What are the obstacles to addressing depressed patients to psychotherapy, in Alsace ? Methods : A cross-sectional, descriptive, epidemiology study was carried out from may to august 2019, amongst general practitioners of the region of Alsace, France. The participants were invited, via email, to answer a questionnaire built after a literary review. Results : 3 main obstacles were found in addressing patients to psychotherapy : the cost of it (75%), the access difficulties (65%), and patients reluctance (62%). Influencing factors were found : practitioners who felt at ease treating depressed patients, who had a positive opinion about psychotherapy, and who had personally benefited from psychotherapy themselves, suggested this treatment more to their depressed patients. Conclusion : Psychotherapy referral is a complex process. Several ways of improvement are to be considered : Regarding GP4s, the quality of their university teaching is crucial, to contribute to familiarize them with psychotherapy. A strengthening of supportive psychotherapy teaching for GP's would be necessary. For patients, destigmatization, which is one of the WHO's main goal since 1998, is to be pursued. Finally, regarding french health's system, an experiment is being done (2018-2020), permitting reimbursed psychotherapy sessions in 3 departments. This is inspired by the UK "Improving Acces to Psychological Therapies" experiment, a project which started in the UK in 2008, with the goal of free cognitive behavioral therapies for patients with anxious and depressed disorders. The hope being the improvement of patient's well being, and a long-term economic benefit overall. The results of this large-scale project are to follow in the future.
