Etude qualitative sur l’annonce du diagnostic de trouble bipolaire : Recueil du ressenti des patients et pistes d’améliorations

Description : Médecine. Psychiatrie, Introduction : le trouble bipolaire est un problème majeur de santé publique avec de nombreuses répercussions médico-psycho-sociales. Il existe malheureusement un délai de cinq à dix ans entre le premier épisode thymique et l'annonce du diagnostic. Cette annonce peut être diversement vécue, jusqu'à parfois constituer un traumatisme chez le patient, alors qu'il s'agit d'une étape déterminante dans le parcours de soins. Le travail de cette thèse consistait à évaluer les pratiques, le ressenti des patients et les améliorations possibles concernant cette annonce diagnostique. Méthode : recherche qualitative par entretiens semi-dirigés, réalisés de février à août 2020, auprès de 10 participants majeurs souffrant d'un trouble bipolaire. Résultats : deux aspects ressortent dans le discours des participants : l'annonce est à la fois source de soulagement et de choc. Ils déplorent principalement le manque d'informations. De nombreuses améliorations ont été proposées : privilégier une annonce en plusieurs fois, plus précoce dans l'évolution de la maladie, en présence d'un proche, avec un médecin expérimenté et humble, qui apporte des explications claires. Ces réflexions correspondent globalement aux préconisations de la HAS concernant l'annonce d'une mauvaise nouvelle. Discussion : la question se pose de la mise en place d'un protocole standardisé, comparable au plan cancer et au protocole EPICES, pour favoriser le respect des recommandations. Concernant les délais d'annonce diagnostique, l'intervention de patients enseignants au cours de la formation des étudiants en DES de psychiatrie permettrait de les sensibiliser davantage à l'annonce du diagnostic de trouble bipolaire. Enfin, il serait intéressant de favoriser le compagnonnage pour que les internes puissent assister à des annonces avec un sénior avant de pouvoir en faire par eux-mêmes., Introduction: Bipolar disorder is a major public health problem with many medico-psycho-social problems. Unfortunately, there is an intervall of five to ten years between the first mood episode and the announcement of the diagnosis. This announcement can be experienced in different ways, sometimes until a trauma for the patient, whereas it is a decisive step in the care pathway. This study aims to evaluate the practices, the feelings of the patients and the possible improvements concerning this diagnostic announcement. Method: Qualitative research by semi-structured interviews, carried out from February to August 2020, with 10 adult participants suffering from bipolar disorder. Results: Two aspects emerge from the participants' speech: the announcement is both a source of relief and shock. They mainly deplore the lack of informations. Many improvements have been proposed: an announcement in several times, earlier in the course of the disease, in the presence of close relations, with an experienced and humble doctor, who provides clear explanations. These reflections overall correspond to HAS's guidelines concerning the announcement of bad news. Discussion: This raises the question of setting up a standardized protocol, similar to the“plan cancer and the protocol SPIKES, for a better observance of guidelines. Concerning delayed diagnosis of bipolar disorder, the intervention of "teaching patients" during the training of DES students in psychiatry would increase their awareness of this announcement. Finally, it would be interesting to promote companionship so that students can be present at announcements with a senior before being able to do it on their own.
