Description : Médecine. The prevention of falls is a major public health issue, half of the adults older than 80 years fall each year in France. A fall can have serious consequences in frail patients, as are most patients hospitalized in acute geriatrics station. Gait disorders, present in more than a third of patients older than 70 years, are predisposing factors for falls. They are accessible to rehabilitation and can be analyzed using tools that collect spatio-temporal parameters of walking, such as the GAITrite® mat. Objective: To find differences between the spatio-temporal gait parameters in patients who fall compared to patients who do not fall in a population of patients hospitalized in acute geriatrics station. Material and methods: In this prospective, observational, single-center study, patients over 75 years hospitalized in acute geriatrics station at the Robertsau hospital were included from June 2019 to February 2020. For each patient, spatio-temporal gait parameters were collected in a single task then in a double task using the GAITrite® mat. The patients were divided into two groups according to whether or not they had a history of fall. The spatio-temporal parameters and demographic characteristics were compared between the two groups and in relation to the general population. Coefficients of variability were also calculated for a few of spatio-temporal gait parameters, then compared between the two groups. Results: 67 patients, with an average age of 85.9 ± 6 years, were included. Seventy percent of patients had cognitive impairment and 66% required technical walking assistance. The patients had multimorbidities (Charlson score on average 7.00 (± 1.87)) and multiple medication (7.04 ± 3.25 drugs per patient). There was significantly more history of stroke (17 vs 2, p = 0.023), HTO (12 vs 0, p = 0.013) and osteoporosis (15 vs 2, p = 0.044) in the group of patients who fall. The mean walking speed was 51.4 ± 19.9 cm/s in non-fallers, and 47.3 ± 17.7 cm/s in fallers (p = 0.539), indicating pathological walking in comparison with the general population of the same age (average 100 cm / s). No association was found between the spatiotemporal gait parameters and fall. The variability of the spatiotemporal gait parameters was comparable in the two groups. Conclusion: In our population, no association was found between the spatiotemporal gait parameters and fall, probably linked to the small number of our study, and to many confounding factors such as the pathogenicity of walking of our patients and their demographic characteristics. A study with a larger number of patients is necessary in order to find changes in spatio-temporal parameters statistically linked to the risk of falls, with the aim of setting up specific rehabilitation protocols.
