Intérêt de l'infiltration de plasma riche en plaquettes (PRP) comme alternative thérapeutique dans le traitement des pathologies mécaniques chroniques périphériques en rhumatologie ambulatoire.

Description : Médecine générale, Introduction : les injections de plasma riche en plaquettes (PRP) apparaissent comme un traitement novateur et une alternative thérapeutique dans les douleurs mécaniques en Rhumatologie. Si leur tolérance et leur innocuité semblent faire consensus, leur efficacité et l’avis des professionnels de santé sont controversés. Compte-tenu de cette ambivalence et afin d’obtenir une opinion basée sur des résultats objectifs issus de notre propre cohorte, nous avons mené un travail dans ces indications. Matériels et Méthodes : nous avons réalisé une étude rétrospective observationnelle monocentrique menée au sein d’un cabinet de Rhumatologie libérale à Strasbourg. Ont été inclus les patients présentant des douleurs mécaniques articulaires périphériques (gonarthrose essentiellement) ou tendineuses (épicondylite, tendinopathie Achilléenne et du moyen fessier surtout) depuis au moins 3 mois. Le critère de jugement principal était l’évaluation visuelle analogique (EVA) de la douleur. Les critères secondaires étaient l’efficacité du PRP en fonction de la pratique ou non d’une auto-kinésithérapie, l’intensité de la douleur per-procédure et la satisfaction globale. Tous les patients ont bénéficié d’une injection de PRP obtenu via le kit Arthrex® (PRP sans anticoagulant ni activateur avec centrifugation unique à 1500 tours/min pendant 5 minutes). Toutes les injections ont été réalisées sous contrôle échographique. Résultats : 105 infiltrations de PRP ont été évaluées entre Juin et Septembre 2019 et réparties en 4 groupes en fonction du délai d’évaluation (6 semaines (16 infiltrations), 3 mois (23), 6 mois (34) et 12 mois (32)). Pour chacun des groupes et pour toutes les indications, on observe une diminution de l’EVA douleur, maximale à 6 mois. La baisse de l’EVA est plus importante chez les patients ayant pratiqué une auto-kinésithérapie dans l’indication « Arthrose » versus ceux qui n’en ont pas pratiqué. Concernant l’indication « Tendinopathie », le PRP semble d’autant plus efficace que l’atteinte est sévère. L’injection de PRP est bien tolérée, et même si le geste est ressenti comme douloureux à l’injection, en particulier pour les indications tendineuses, il n’y a pas de mémoire de la douleur. Discussion/conclusion : le PRP s’est révélé efficace avec une baisse de l’EVA à court, moyen et long terme. Il est intéressant de souligner l’effet potentialisateur de l’auto-kinésithérapie en association avec le PRP dans l’indication « Arthrose ». Ainsi le PRP doit, à notre avis, être considéré comme un traitement adjuvant aux thérapeutiques habituelles et ainsi rentrer dans l’arsenal thérapeutique de la prise en charge des pathologies articulaires et tendineuses mécaniques., Introduction : Platelet-rich plasma (PRP) injections seem to be an innovative treatment and an alternative therapy for mechanical pain in Rheumatology. If there is a consensus on their tolerance and safety, their efficacy and health professional’s opinion are controversial. Due to this ambivalence and to obtain an opinion based on objective results from our own cohort, we have conducted a study on these indications. Materials and Methods : We have conducted a retrospective observational and monocentric study in a Rheumatological private medical practice in Strasbourg. Patients were included if they had articular mechanical peripheral pain (mainly knee osteoarthritis) or tendinous pain (mostly tennis elbow, Achilles tendinopathy and gluteus medius tendinopathy) for at least 3 months. The principal judgment criteria was the pain visual analogic scale (VAS). The secondary criteria were the analysis of the influence of a self-reeducation after the injection, the pain intensity of the injection and the global satisfaction. All the patients benefited of a single injection of PRP which was obtained by the Arthrex® kit (PRP without anticoagulant nor activator with a single centrifugation at 1500 turns/min for 5 minutes). All the injections were ultrasound-guided. Results : 105 PRP injections were evaluated between June and September 2019 and divided into 4 groups according to the date of evaluation (6 weeks (16 injections), 3 months (23), 6 months (34) and 12 months (32)). For each group and for all the indications we observed a pain reduction, that was maximal at 6 months. Pain reduction was more important in patients who had practiced self-reeducation in the indication “Osteoarthritis” versus those who hadn’t. In the “Tendinopathy” indication it seems that the more severe the damage, the more efficient the PRP was. The PRP injection was well tolerated, and even if it feels painful at the moment of the injection, in particular in tendinous locations, there is no memory of pain. Discussion/conclusion : PRP has showed its efficacy in pain reduction at short, middle and long term. It’s interesting to highlight the potentiator effect of self-reeducation in association with PRP in “Osteoarthritis” indications. Thus PRP, from our point of view, should be considered as an adjuvant treatment and be a part of the therapeutic arsenal for treating articular and tendinous mechanical pathologies.
