L’endocardite aiguë infectieuse, un diagnostic à ne pas méconnaître en service d'accueil des urgences

Description : Médecine d’urgence, Introduction : l’endocardite infectieuse est une pathologie dont la mortalité reste élevée. Les endocardites liées aux soins et aux pathologies chroniques prédominent de nos jours. La présentation polymorphe de la pathologie peut induire un retard diagnostique. L’objectif principal de l’étude est d’évaluer si le diagnostic d’endocardite infectieuse est suffisamment évoqué au service d’accueil des urgences (SAU). Méthode : tous les patients atteints d’endocardite infectieuse entre le 1er Janvier 2011 et le 31 Décembre 2017 étant passés par les urgences puis hospitalisés à l’hôpital de Mulhouse ont été inclus dans cette étude rétrospective monocentrique. Résultats : 44 patients ont été inclus. Le diagnostic d’endocardite a été porté au SAU chez seulement 10 patients alors que les signes étaient déjà présents chez 25 de ces patients. Ce retard diagnostique a engendré un allongement du délai d’initiation de l’antibiothérapie et un risque majeur d’augmentation de la mortalité. Conclusion : dans notre étude, le diagnostic d’endocardite est insuffisamment évoqué. Ce retard diagnostique impacte sur le plan thérapeutique et sur le pronostic des patients., Introduction: Infectious endocarditis is a pathology whose mortality remains high. The main objective of the study is to assess whether the diagnosis of infectious endocarditis is sufficiently mentioned in the emergency department (ED). Method: All patients with infectious endocarditis between January 1, 2011 and December 31, 2017 who went through the emergency room and then hospitalized at Mulhouse hospital were included in this single-center retrospective study. Results: 44 patients were included. The diagnosis of endocarditis was made by the ED in only 10 patients when the signs were already present in 25 of these patients. This diagnostic delay has resulted in a longer time to initiation of antibiotic therapy and a major risk of increased mortality. Conclusion: In our study, the diagnosis of endocarditis is insufficiently evoked. This diagnostic delay has an impact on the therapeutic level and on the prognosis of patients.
