<dcterms:abstract xml:lang="fr">Les travaux menés au cours de cette thèse se sont articulés autour de l'étude des phénotypes de l'haptoglobine (Hp) humaine, de leur purification, de leurs interactions avec l'hémoglobine (Hb) et des implications in vitro dans le stress oxydant induit par l'Hb extra-érythrocytaire. Les résultats ont été appliqués à l'autotransfusion utilisant la récupération sanguine péri-opératoire. Nous avons mis au point un protocole de phénotypage de l'Hp humaine par électrophorèse en gel de polyacrylamide, que nous avons appliqué à l'étude de la répartition des trois phénotypes dans une population de patients hospitalisés. Par la suite, nous avons purifié par FPLC l'Hp à partir de plasmas humains phénotypés (échange d'ions et exclusion-diffusion). L'interaction de chacun des trois phénotypes purifiés d'Hp avec l'Hb humaine a été étudiée en spectrophotométrie, à travers le pouvoir peroxydasique du complexe Hp-Hb. Les résultats obtenus ont montré que l'affinité de Hp 1-1 pour l'Hb était supérieure à celle de Hp 1-2, qui était elle-même supérieure à celle de Hp 2-2. La constatation du fait que l'interaction de l'Hp avec l'Hb était exothermique nous a conduit à réaliser des études complémentaires en microcalorimétrie. Dans une étude clinique, nous avons démontré que l'affinité de l'Hp pour l'Hb pouvait avoir pour conséquence une sous-estimation de la concentration réelle d'Hp présente dans des spécimens de patients. Cette interférence, non dépendante du phénotype, a été observée lorsque l'Hp était déterminée en immuno-néphélémétrie. Par la suite, nous avons étudié les effets délétères de l'Hb extra-érythrocytaire sur le globule rouge in vitro sur deux modèles. Le premier a utilisé des préparations de membranes de globules rouges, avec suivi de la peroxydation lipidique par la formation de diènes conjugués et des substances réagissant avec l'acide thiobarbiturique (TBARS). Le deuxième modèle a utilisé des globules rouges intacts, maintenus dans un milieu de conservation (SAGM), avec suivi du potassium et de l'activité lactate déshydrogénase libérés, comme marqueurs d'atteinte de l'intégrité membranaire. L'effet protecteur de chacun des trois phénotypes de l'Hp vis-à-vis du stress oxydant induit par l'Hb extra-érythrocytaire a été étudié sur ces deux modèles. Les résultats ont mis en évidence le rôle anti-oxydant de l'Hp et ont démontré qu il existait des différences significatives selon le phénotype, l'effet protecteur de Hp 1-1 étant supérieur à celui de Hp 1-2, qui était lui-même supérieur à celui de Hp 2-2. Enfin, nous avons évalué la qualité des poches de sang destinées à être re-transfusées lors de la récupération sanguine péri-opératoire et les conséquences de l'hémolyse en terme de stress oxydant sur le globule rouge. Nos résultats ont montré que les poches destinées à être re-transfusées contenaient des concentrations élevées d'Hb libre. Un stress oxydant a été mis en évidence au niveau des globules rouges des poches, mais également dans la circulation générale des patients après transfusion. Ce stress oxydant s'accompagnait d'une baisse des concentrations plasmatiques en thiols non protéiques et en Hp, ainsi que d'une altération des propriétés mécaniques des membranes de globules rouges. L'ensemble de ces résultats souligne l'importance de maîtriser l'hémolyse lors de la récupération sanguine péri-opératoire, pour limiter ses effets délétères.</dcterms:abstract>
<dcterms:abstract xml:lang="en">This thesis focuses on the study of human haptoglobin phenotypes (Hp), their purification, their interactions with hemoglobin (Hb), and on in vitro implications in oxidative stress induced by fre Hb. The results were applied to the autologous transfusion using perioperative blood salvage. We developed a protocol of human Hp phenotyping using polyacrylamide gel electrophoresis, which was applied to the study of the distribution of the three phenotypes in patients in hospital. Then, we purified the phenotyped human plasmas by FPLC (ion exchange chromatography and gel filtration). The interaction of the three purified Hp phenotypes with human Hb was studied in spectrophotometry, through the peroxidase activity of Hp-Hb complex. The results obtained showed that the affinity of Hp 1-1 for Hb was higher than that of Hp 1-2, which was itself higher than that of Hp 2-2. The interaction of Hp with Hb was proved to be exothermic, which allowed us to conduct complementary studies in microcalorimetry. In a clinical study, we found that the affinity of Hp for Hb could lead to underevaluating the real Hp concentrations in specimens of patients. This interference, which was independent of the phenotype, was observed when Hp was determined by immuno-nephelemetry. Then, we studied the deleterious effects of extracellular Hb on red blood cells in vitro on two models. The first one used preparations of red blood cells membranes, in which we followed the lipid peroxidation by combined dienes formation and substances reacting with thiobarbituric acid (TBARS). The second model used intact red blood cells, maintained in a conservation medium (SAGM), with follow-up of released potassium and lactate dehydrogenase activity, as markers of attack on membrane integrity. The protective effect of each of the three phenotypes of Hp with respect to oxidative stress induced by extracellular Hb was studied on these two models. The results confirmed the antioxidant role of Hp and showed that there were significant differences according to the phenotype, the protective effect of Hp 1-1 being higher than that of Hp 1-2, which was itself higher than that of Hp 2-2. Finally, we evaluated the quality of the blood-pack units during perioperative autologous transfusion and the consequences of hemolysis in terms of oxidative stress on red blood cells. Our results indicated that the blood units intended to be transfused contained high concentrations of fre Hb. An oxidative stress was found on the red blood cells contained in the blood bags and in the general circulation of patients after transfusion. This oxidative stress was accompanied by a decrease in plasmatic non-protein thiols and Hp concentrations, as well as a deterioration of the mechanical properties of red blood cells membranes. All the results showed the importance of hemolysis control during perioperative blood salvage, in order to limit its deleterious effects.</dcterms:abstract>
