Description : Médecine générale, Introduction : Le sepsis est un enjeu majeur de santé publique, avec une mortalité estimée à 20%. La survie du patient dépend en grande partie des délais de prise en charge thérapeutique, en particulier de la rapidité de mise en place d'une antibiothérapie. Le qSOFA est un outil très simple proposé en 2016 lors de la Surviving Sepsis Campaign pour identifier les patients à risque de décès intrahospitalier lié à une infection. Dans cette étude, nous avions pour objectif principal de déterminer l'impact de la mise en place du qSOFA à l'accueil des urgences sur le délai d'initiation d'une antibiothérapie. Un des objectifs secondaires était d'améliorer la mesure de la fréquence respiratoire au poste IOA. Matériel et méthode : Ce travail est une étude des pratiques professionnelles sous la forme d'une analyse rétrospective de type « avant-après ». Une comparaison a été établie entre deux périodes de 4 mois (décembre 2018 à mars 2019 et décembre 2019 à mars 2020), pour les patients ayant consulté aux urgences pour un sepsis avec un score qSOFA ≥2 (calculé dans le premier groupe, mesuré au poste IOA dans le deuxième groupe). Résultats : 109 patients ont été inclus dans la phase avant et 118 dans la phase après. Le délai médian d'introduction d'une antibiothérapie n'était pas amélioré après mise en place du qSOFA, avec une durée de 3h44 (Q1 2h20, Q3 5h51) dans le groupe « avant » et de 4h23 (Q1 2h40, Q3 7h18) dans le groupe « après » (p=0,08). La fréquence respiratoire a été mesurée au poste IOA pour 58% des patients du groupe avant et 92% du groupe après (p-0,01). Conclusion : Au vu de la faible sensibilité de ce score, devant les résultats de cette étude et après revue de la littérature scientifique, le qSOFA n'a pas prouvé son utilité dans l'aide au triage, et son utilisation systématique n'est pas à recommander. La fréquence respiratoire est en revanche un bon indicateur de gravité et sa mesure au poste IOA doit être pérennisée. La mise en place d'une filière sepsis déclenchée par le médecin d'accueil des urgences serait intéressante pour tenter de réduire les délais d'introduction d'une antibiothérapie, qui restent trop longs en cas d'infection grave, Introduction : Sepsis is a major public health issue, with an estimated mortality of 20%. A comparison was made between two 4-month periods (December 2018 to March 2019 and December 2019 to March 2020), for patients who visited the emergency department for sepsis with a qSOFA score ≥2 (calculated in the first group, measured at the emergency reception in the second group). Results : 109 patients were included in the before phase and 118 in the after phase. Median time to initiation of antibiotic therapy was not improved after qSOFA implementation, with a duration of 3h44 (Q1 2h20, Q3 5h51) in the "before" group and 4h23 (Q1 2h40, Q3 7h18) in the "after" group (p=0.08). The implementation of a sepsis network triggered by the emergency physician would be interesting to try to reduce the delay in the introduction of antibiotic therapy, which remains too long in cases of serious infection
