L’angioscanner pulmonaire peut-il aggraver un oedème aigu pulmonaire ?

Description : Médecine (radiodiagnostic et imagerie médicale), Le but de ce travail est d'analyser le potentiel impact délétère de l'angioscanner pulmonaire chez des patients en OAP. 3928 patients consécutifs ayant bénéficié d'un angioscanner pour suspicion d’embolie pulmonaire ont été rétrospectivement inclus. Le protocole consistait en 50mL de ioméprol 400mg/mL injecté à 3,5 m/L sec suivi par 30 mL de sérum physiologique à 3mL/sec. Les dossiers ont été revus par un même opérateur à la recherche de signes scanographiques d'insuffisance cardiaque gauche (ICG). Le dossier électronique des patients ICG+ avec dosage des BNP-100 à été revu à la recherche de complications imputables à l'examen et la mortalité à 30 jours. 383 patients étaient ICG+, dont 296 avec BNP-100. La mortalité à 30 jours était de 15 patients soit 5% et aucun cas n'étant imputable à l'angioscanner. Une seule aggravation (0,3%) d’OAP suite à l'angioscanner a été relevée. Sur une large série consécutive de 3928 patients ayant bénéficié d'un angioscanner, le pourcentage d’OAP est significatif (296 patients, 7,5%) et la réalisation de l’examen semble sure dans cette sous population avec absence de decès imputable et une seule complication à J30 (0,3%)., The aim of this work is to analyze the potential deleterious impact of CT pulmonary angiography (CTPA) in acute pulmonary oedema (APO) patients. 3928 consecutive patients who received CTPA for suspected pulmonary embolism were retrospectively included. The protocol consisted of 50mL of iomeprol 400mg/mL injected at 3.5mL/sec followed by 30 mL of saline at 3mL/sec. The records were reviewed by the same operator for CT signs of left heart failure (LHF). The LHF+ electronic patient record with BNP-100 was reviewed for examination complications and mortality at 30 days. There were 383 LHF+ patients, 296 of whom had BNP-100. Mortality at 30 days was 15 patients (5%) and no cases were attributable to CTPA. Only one worsening of APO following CTPA was noted (0,3%). In a large consecutive series of 3928 patients who received CTPA, the percentage of APO was significant (296 patients, 7.5%) and the examination appeared to be safe in this subpopulation with no attributable deaths and only one complication at D30 (0.3%).
