Influence de la bonne adaptation de la thérapie anti infectieuse peropératoire sur les complications postopératoires d'une duodénopancréatectomie pour tumeur péri-ampullaire

Description : Médecine (anesthésie-réanimation), La duodénopancréatectomie (DP) est grevée d’une morbidité importante. Les complications infectieuses en représentent une part importante, dont le drainage biliaire préopératoire (DBP) est un des principaux facteurs de risque en favorisant la contamination biliaire peropératoire. Nous avons décrit la morbiortalité liée à cette intervention et l’influence de la bonne adaptation de la thérapie anti-infectieuse sur le taux de complications postopératoires. Nous avons réalisé une étude rétrospective monocentrique incluant 106 patients opérés d’une DP entre 2012 et 2018. 65 patients (61%) ont bénéficié d’un DBP. Le taux de mortalité à J90 était de 12%. Nous notions un taux de complications postopératoires non infectieuses de 90% et infectieuses de 63%. La bonne adaptation de la thérapie anti-infectieuse peropératoire chez les patients opérés d’une DP avec DBP n’a pas d’influence sur les complications postopératoires, Pancreaticoduodenectomy (DP) has a high rate of morbidity. Infectious complications represent a major part of these, for which preoperative biliary drainage (PBD) is one of the main risk factors by promoting perioperative biliary contamination. We described the mortality and morbidity of DP and the influence of the adaptation of the perioperative anti-infectious therapy on the postoperative complications. 106 patients who underwent DP from 2012 to 2018 were identified. 65 patients (61%) underwent PBD. The rate of 90-day mortality was 12%. The postoperative rate of non-infectious complications was 90% and the rate of infectious complications was 63%. The adaptation of the perioperative anti-infectious therapy didn’t decrease significantly the rate of postoperative complications.
