Description : Médecine (pneumologie), Le cancer broncho-pulmonaire primitif est la première cause de mortalité d'origine néoplasique dans le monde. Le traitement des stades métastatiques repose sur la chimiothérapie, l'immunothérapie ou les thérapies ciblées. Le concept de thérapies ciblées repose sur l'identification d'altérations moléculaires à l'origine de la survenue des cancers. Les inhibiteurs de tyrosine kinase (TKI) qui ciblent les protéines codées par ces oncogènes mutés se sont révélés supérieurs à la chimiothérapie. Un des principaux enjeux actuels en oncologie thoracique est donc d'identifier de nouvelles altérations moléculaires accessibles à une thérapie ciblée. La protéine MET est un récepteur tyrosine kinase exprimé à la surface des cellules épithéliales. Des dérégulations de MET et de son ligand sont retrouvées dans plusieurs cancers chez l'Homme, notamment le cancer broncho-pulmonaire et sont associées à un mauvais pronostic. Ce travail est une étude rétrospective multicentrique, décrivant les caractéristiques démographiques et cliniques de patients mutés dans le domaine tyrosine kinase de MET. Nous avons identifié au CHRU de Strasbourg et de Lille des patients atteints de CBNPC porteurs d’une mutation du domaine tyrosine kinase de MET, cependant les conséquences fonctionnelles de telles mutations sont inconnues dans ce contexte. L’enjeu pour ces patients sera de déterminer, via des études pré-cliniques, si les mutations identifiées sont activatrices, et si elles sont sensibles aux inhibiteurs de MET. Auquel cas, les patients mutés dans le domaine kinase de MET pourraient bénéficier de thérapies ciblées, Primary bronchopulmonary cancer is the leading cause of neoplastic death in the world. Treatment of the metastatic stages is based on chemotherapy, immunotherapy or targeted therapies. The concept of targeted therapies is based on the identification of molecular alterations at the origin of the occurrence of cancers. Tyrosine kinase inhibitors (TKIs) that target the proteins encoded by these mutated oncogenes have been shown to be superior to chemotherapy. One of the main current challenges in thoracic oncology is therefore to identify new molecular alterations accessible to targeted therapy. The MET protein is a receptor tyrosine kinase expressed on the surface of epithelial cells. Deregulations of MET and its ligand are found in several cancers in humans, in particular bronchopulmonary cancer and are associated with a poor prognosis.This work is a retrospective multicenter study, describing the demographic and clinical characteristics of patients mutated in the tyrosine kinase domain of MET.We have identified patients with NSCLC at the Strasbourg and Lille university hospitals with a mutation in the tyrosine kinase domain of MET, however the functional consequences of such mutations are unknown in this context.The challenge for these patients will be to determine, via pre-clinical studies, whether the identified mutations are activating, and whether they are sensitive to MET inhibitors. In which case, patients mutated in the kinase domain of MET could benefit from targeted therapies
