Évaluation rétrospective de l’intérêt d’une chimiothérapie adjuvante chez les patients atteints d’un adénocarcinome rectal, non métastatique traité par radiochimiothérapie puis chirurgie

Description : Médecine (gastro-entérologie, hépatologie), Introduction : Le traitement du cancer du rectum localement avancé non métastatique repose sur l’association radiochimiothérapie suivie d’une chirurgie avec exérèse du mesorectum. Par contre, l’administration d’un traitement adjuvant, chez les stades II et III sur le pièce opératoire n’est actuellement pas consensuelle. C’est dans ce cadre que nous avons analysé de façon rétrospective la survie sans récidive (SSR) et la survie globale (SG) ainsi que leurs facteurs prédictifs dans une population de patients ayant bénéficié de la séquence thérapeutique radiochimiothérapie - chirurgie suivie ou non d’un traitement post-opératoire pour un cancer du rectum à Strasbourg. Patients et Méthodes : Il s’agissait d’une étude rétrospective multicentrique menée du 01/01/06 au 31/12/16 chez des patients ayant tous bénéficié d’une radiothérapie au Centre de Lutte Contre le Cancer Paul Strauss associée à une chimiothérapie suivie d’une chirurgie effectuée dans différents centres pour un cancer du rectum localement avancé, non métastatique. Les données cliniques au diagnostic, les caractéristiques des traitements néoadjuvant et adjuvant, du traitement chirurgical, de la pièce opératoire ainsi que les données de survie étaient recueillies. Résultats : 331 patients ont été inclus (231 hommes, 100 femmes, âge médian 64 (32-87) ans) avec un suivi médian de 64,4 mois [60,4-69,3]. 166 patients (50,2 %) ont bénéficié d’un traitement adjuvant alors que 165 (49,8 %) n’en avaient pas reçu. 86 patients sont décédés et 83 ont présenté une récidive. La médiane de SSR n’était pas atteinte et la médiane de SG était de 126,3 mois [114,7-NA]. La SSR était de 114,7 mois [90,30-130,5] dans le bras chimiothérapie adjuvante versus de 145,9 mois [121,00-145,9] dans le bras sans chimiothérapie (p = 0,01). La SG n’était pas différente (119,1 mois [91,1–NA] versus 126,3 mois [109,1-NA] ; p = 0,152). Chez les patients avec une tumeur de stade II sur la pièce opératoire, un traitement adjuvant n’améliorait ni la SSR (médianes non atteintes ; p = 0,579) ni la SG (127,5 mois [100-NA] ; p = 0,152). A l’inverse, chez les patients de stade III, la chimiothérapie adjuvante améliorait significativement la survie globale (91 mois [65,4-NA] versus 36,4 mois [23,9-44,2] ; p = 0,005). En analyse multivariée, les facteurs significatifs prédictifs de survie étaient l’atteinte ganglionnaire post-opératoire et le statut OMS. Conclusion : Notre travail suggère que l’administration d’une chimiothérapie adjuvante chez les patients avec une tumeur de stade II post-opératoire n’améliorerait pas la survie et ce quelque soit le statut ganglionnaire initial. Toutefois, elle garderait un intérêt chez les patients avec une tumeur de stade III en améliorant la SG., Introduction: Treatment of locally advanced non-metastatic rectal cancer is based on the association of chemoradiotherapy followed by total mesorectal excision surgery. On the other hand, the administration of an adjuvant treatment, especially in stage II and III postoperatively, is still to determine. It is within this framework that we have analyzed retrospectively the disease-free survival (DFS), the overall survival (OS) and their predictive factors in a population of patients beneficiated from a chemoradiotherapy and a surgery followed or not by adjuvant chemotherapy for a rectal cancer in Strasbourg. Patients et Methods: This work was a retrospective multicenter study conducted from January 1, 2006 to December 31, 2016 in patients beneficiated from radiotherapy in the Centre de Lutte Contre le Cancer Paul Strauss in Strasbourg associated with chemotherapy and followed by a surgery performed in several centers for a locally advanced non-metastatic rectal cancer. Clinical data at the time of the diagnosis, characteristics of the neoadjuvant and adjuvant treatments, of the surgery and of the surgical specimen features and survival data were collected. Results: 331 patients were included (231 men, 100 women, median age 64 (32-87) years) with a median follow-up of 64.4 months [60.4-69.3]. 166 patients (50.2%) had adjuvant chemotherapy whereas 165 (49.8%) had no adjuvant treatment and were observed. 86 patients died and 83 had recurrence. Median of DFS was not reached and median of OS was 126.3 months [114.7-NA]. The DFS was 114.7 months [90.30-130.5] in patients with chemotherapy versus 145.9 months [121.0-145.9] in patients without chemotherapy (p=0.01). The OS was not different (119.1 months [91.1–NA] versus 126.3 months [109.1-NA]; p=0.152). In patients with stage II postoperatively tumor, adjuvant treatment did not improved the DFS (median not reached; p=0.579) or the OS (127.5 months [100-NA]; p=0.152). Conversely, in patients with stage III tumor, adjuvant chemotherapy improved significantly the OS (91 months [65.4-NA] versus 36.4 months [23.9-44.2]; p=0.005). In multivariate analysis, significant predictor factors of survival were postoperative lymph node involvement and PS status. Conclusion: Our work suggests that the administration of adjuvant chemotherapy would not improve the survival in patients with stage II postoperatively tumor, whatever the initial lymph node status or the T status. However, it would still be interested in patients with a stage III tumor by improving the OS., Thèses et écrits académiques
