Description : Médecine (radiodiagnostic et imagerie médicale), Introduction : Les progrès techniques et la préoccupation croissante au sujet de l’exposition aux rayonnements ionisants ont conduit au développement d’algorithmes de reconstruction d’images utilisant la technologie de Deep Learning (DLR). Peu d’études à ce jour ont étudié l’impact des algorithmes de DLR en terme de réduction de dose en vie réelle et à grande échelle. Matériels et méthodes : Notre étude est rétrospective et monocentrique. Elle porte sur une population globale de 10306 patients ayant réalisé un examen sur un scanner utilisant un algorithme de reconstruction par DLR (CT-DLR) ou un algorithme de reconstruction par reconstruction itérative (CT-IR) selon un parmi quatre protocoles d’imagerie différents. La qualité des images a été évaluée à la fois qualitativement par un opérateur et quantitativement par la mesure de rapports Signal sur bruit et Contraste sur bruit standardisés. Résultats : La diminution d’irradiation globale du groupe CT-DLR par rapport au groupe CT-IR est estimée à environ 20%, plus marquée pour le protocole « Thorax standard » (22%) et moins marquée pour le protocole « crâne standard » (19%). Le groupe CT-DLR présente une qualité diagnostique d’images supérieure à celle du groupe CT-IR. Conclusion : Ces résultats sont en accord avec ceux déjà publiés dans la littérature : les algorithmes de DLR permettent une diminution notable de la dose d’irradiation tout en obtenant une qualité d’image supérieure par rapport aux algorithmes d’IR. It concerns a global population of 10306 patients having performed an examination on a scanner using a DLR reconstruction algorithm (CT-DLR) or an iterative reconstruction algorithm (CT-ILR) according to one of four different imaging protocols. Results: The global irradiation reduction of the CT-DLR group compared to the CT-IR group is estimated to be about 20%, more marked for the "Standard Thorax" protocol (22%) and less marked for the "standard brain" protocol (19%). The CT-DLR group had a higher diagnostic image quality than the CT-IR group. Conclusion: These results are in agreement with those already published in the literature: the DLR algorithms allow a significant reduction of the irradiation dose while obtaining a superior image quality compared to IR algorithms.
