Description : Médecine (médecine générale), Objectif : Décrire les modalités de prescriptions de corticothérapie systémique au cours des 4 premières semaines d’une infection symptomatique au SARS-CoV-2 par les médecins généralistes alsaciens. Secondairement, mettre en évidences les déterminants de ces modalités de prescription, décrire leurs évolutions pendant et après la première vague et décrire les différentes sources d’informations ayant pu intervenir dans l’évolution des prescriptions. Introduction : La covid-19 a entrainé une crise sanitaire mondiale mais reste une maladie virale respiratoire essentiellement bénigne et prise en charge principalement en médecine de premiers recours avec encore beaucoup d’interrogations sur les moyens thérapeutiques disponibles. Les corticothérapies systémiques étant régulièrement utilisées en médecine générale dans les maladies virales respiratoires, nous nous sommes donc intéressés à leur utilisation dans la covid-19 Méthodes : Enquête de pratique par une étude descriptive quantitative réalisée par auto-questionnaire diffusé sur internet et destiné aux médecins généralistes alsaciens libéraux ou salariés. Résultats : 69 participations 37 retenues et analysées avec : 48,6% des médecins interrogés qui ont prescrit une corticothérapie systémique pour la covid-19, majoritairement pour des signes respiratoires de type toux asthmatiforme (83,3%) et en cas d’oxygénothérapie concomitante (55,5%). Les molécules les plus utilisées étaient la prednisolone et la prednisone à la dose de 1 mg/Kg. Une évolution des pratiques des participants après l’arrivée des recommandations officielles a été constaté chez 66,6% des prescripteurs de corticothérapie. Ce changement dans les indications s’accompagne également d’une majoration de l’utilisation des corticoïdes avec 55,5% des prescripteurs de corticothérapie qui pensent en avoir prescrit plus souvent après la première vague, 33,3 % pensent avoir été stable et 11,1% pensent en avoir prescrit moins souvent après la première vague. Les participants s’étaient informés principalement via les institutions et organismes officiels, par échanges de pairs et enfin via leurs recherches personnelles. Conclusion : Cette étude montre une utilisation non négligeable de corticothérapie systémique pour la covid- 19 en ambulatoire, en accord avec les recommandations officielles mais aussi en dehors de celles-ci. Nous avons aussi constaté une évolution dans les indications et une majoration de la fréquence d’utilisation des corticoïdes après la sortie des recommandations officielles. Les participants de notre étude ayant prescrits des corticoïdes dans le contexte étudié́ semblaient être guidé par un principe de précaution et un raisonnement par analogie avec les autres infections virales respiratoires prise en charge en ambulatoire., Objective : To describe the modalities of systemic corticosteroid prescriptions during the first 4 weeks of a symptomatic infection with SARS-CoV 2 by Alsatian general practitioners. Secondarily, highlight the determinants of these prescription methods, descibe their evolution during and after the first wave and describe the different sources of information that may have intervened in the evolution of prescriptions. Introduction : Covid-19 has caused a global health crisis but is an essentially benign respiratory viral disease and is treated mainly in primary care medicine with still many questions about the therapeutic means available. The systemic corticosteroids being regularly used in general medicine in the respiratory viral diseases, so we were interessed in their use in Covid-19 Methods : Survey of practice by a quantitative descriptive study carried out by self-questionnaire disseminated on the Internet and intended for general practitioners in Alsace who are liberal or salaried. Results : 69 participations 37 retained and analyzed with : 48.6% of the doctors questionned who prescibed systemic corticosteroid therapy for Covid-19, mainly for resoiratory signs asthma-like cough (83.3%) and in case of concomittant oxygen therapy (55%). The most used molecules were prednosolone and prednisone at a dose of 1 mg/Kg. An evolution of the practices of the participants after the arrival of the recommandations official was found in 66.6% of corticosteroid prescibers. This change in the indications is also accompanied by an increase in the use of corticosteroids with 55.5% of corticosteroid prescribers who think they have prescribed them more often after the first wave, 33.3% think they have been stable and 11.1% think they have prescribed it less often after the first wave. The participants had obtained information mainly through official institutions and organizations, through peer exchanges and finally through their personnal research. Conclusion : This study shows a non-negligible use of systemic corticosteroid therapy for covid-19 on an out patient basis, in accordance with official recommandations but also outside them. We have also observed an evolution in the indications and an increase in the frequency of use of corticosteroids after the publication of the official recommendations. The participants in our study who prescribed corticosteroids in the context studied seemed to be guided by a precautionary and reasoning by analogy with other viral infections treated on an outpatient basis., Thèses et écrits académiques
