Description : Médecine (hépatologie et gastroentérologie), L'hépatite alcoolique aiguë (HAA) est une pathologie fréquente avec une mortalité élevée, qui se complique souvent d'infections. Leur diagnostic est parfois difficile. Cette étude rétroscpective menée sur 60 patients au CHU de Strasbourg a cherché à identifier la présence de facteurs prédictifs d'infections dans une population de patients qui présentent une HAA sévère prouvée histologiquement et pour lesquels un traitement par corticoïdes a été introduit. Les infections précoces correspondent à celles développées entre la date d'admision et la date d'introduction du traitement. L'analyse multivariée a montré une association statistiquement significative entre la présence d'une cholestase ductulaire et le développement d'une infection précoce, OR 17,89 (IC 95% 2,77 - 207,3) (p= 0,0075). Il n'a pas été retrouvé d'association avec l'importance de l'infiltrat inflammatoire au sein du parenchyme hépatique. La détection d'une cholestase ductulaire pourrait ainsi permettre d'améliorer les prescriptions d'antibiotiques chez les patients qui présentent une HAA sévère et pour lesquels le diagnosctic d'infection est incertain., Alcoholic hepatitis (AH) is a frequent pathology with a high mortality rate, which is often complicated with infections. This retrospective study included 60 patients in Strasbourg's Hospital, to identify predictive factors for infections in a population of patients with histologically proven severe AH and for whom treatment with steroids had been introduced. Early infections were defined as those that developed between the date of admission and the introduction of the treatment. The multivariate analysis showed a statistically significant association between the presence of ductular cholestasis and the development of early infection, OR 17.89 (95% CI 2.77 - 207.3) (p= 0.0075). No association was found with the extent of inflammatory infiltrate within the liver parenchyma. The detection of ductular cholestasis could thus improve antibiotic prescriptions in patients with severe AH for whom the diagnosis of infection is uncertain., Thèses et écrits académiques
