Pourtant elle reste encore trop tardive ou inexistante. Notre étude exploratoire a donc pour but d'évaluer l'état de connaissance des internes et jeunes médecins (5ans post internat thésés ou non) de médecine générale (soins primaires) sur les moyens d'identifier une situation palliative pour un patient, dès l'instant où la situation se présente comme telle. Méthode : Il s'agit d'une étude observationnelle, prospective, quantitative et nationale. L'enquête a été réalisé de mai à juillet 2022 à l'aide d'un auto-questionnaire élaboré à partir des recommandations HAS de 2016 sur « la démarche palliative ». Résultats : 71 internes ou jeunes médecins de médecine générale et de 14 villes différentes étaient représentés. Une majorité des internes (63%) n'ont pas fait de stage en USP ou EMSP et n'ont pas eu d'enseignement de soins palliatifs pendant leur internat (54%). Au total, 40% des internes n'ont eu ni enseignement ni stage en soins palliatifs pendant leur internat. Les connaissances générales en soins palliatifs sont bonnes. En revanche, en ce qui concerne les outils d'identification, uniquement 7% des internes connaissent la SPICT et aucun ne l'utilise réellement. Ce taux atteint 20% pour les internes étant passés en SP. L'outil Pallia 10 est mieux connu, par 25% des internes (et 50% des internes passés en SP) mais uniquement 6% (12% pour les internes passés en SP) qui l'utilisent. Concernant les cas cliniques, les réponses restent très aléatoires selon les situations, même si les internes passés en soins palliatifs ont tendance à mieux identifier les stades de prise en charge. Conclusion : Cette étude suggère la nécessité d'un enseignement spécifique traitant des soins palliatifs au cours de l'internat de médecine générale. Une formation à la fois théorique et pratique semble être la méthode la plus efficace, Study of the the incentives and obstacles in the identification of a palliative situation within a sample of interns or young doctors (<5 years post-internship) of general practitioners Introduction: The benefits of identifying a palliative situation are numerous. Method : This is an observational, prospective, quantitative and national study. The survey was carried out from May to July 2022 using a self-administered questionnaire based on the 2016 HAS recommendations on "the palliative approach". Results : 71 interns or young general medicine physicians from 14 different cities were represented. A majority of interns (63%) did not do an internship in USP or EMSP and did not have palliative care teaching during their internship (54%). In total, 40% of interns had neither teaching nor internship in palliative care during their internship. Despite, general knowledge of palliative care is good. On the other hand, with regard to identification tools, only 7% of interns are aware of SPICT and none actually use it. This rate reaches 20% for interns who have done an internship in palliative care. The Pallia 10 tool is better known, by 25% of interns (and 50% of interns in SP) but only 6% (12% for interns in SP) who use it. Concerning the clinical cases, the answers remain very random according to the situations, even if the interns who have done an internship in palliative care tend to better identify the stages of care. Conclusions : This study suggests the need for specific teaching dealing with palliative care during general medicine studies.
