Description : Médecine (Neurologie), Introduction: L’évolution de l’arsenal thérapeutique, des critères diagnostiques et des recommandations concernant le traitement dans la sclérose en plaques (SEP) ont conduit à une modification des pratiques des neurologues concernant la prescription d’un traitement de fond dans cette maladie. Notre étude s’intéresse à l’évolution des prescriptions d’un traitement de fond dans la SEP rémittente-récurrente (RR) par les neurologues ayant une activité libérale, hospitalière ou en centre expert, entre 2007 (apparition du Natalizumab) et 2022 (pratique actuelle). Méthodes: Nous avons réalisé, à l’aide de la base de données EDMUS, une étude rétrospective multicentrique, descriptive concernant la prescription d’un traitement de fond chez des patients ayant une SEP RR en Alsace, ayant débuté entre janvier 2007 et juillet 2022 et dont une IRM cérébrale était disponible dans les 6 mois après le début de la maladie. Les traitements suivants étaient considérés comme de haute efficacité: fingolimod, cladribine, rituximab, ocrelizumab, ofatumumab et natalizumab. Résultats: Dans notre étude, portant sur 1218 patients, concernant le 1er traitement de fond de la SEP, le délai de prescription est passé d’environ 1 an à moins de 4 mois de 2007 à 2010 et la période la plus récente de 2018 à 2022. Il existait une proportion de traitements de haute efficacité plus élevée sur la période récente par rapport aux périodes antérieures, avec une modification du profil des patients recevant d’emblée un traitement de fond de haute efficacité, plus âgés et moins sévères sur 64 le plan clinique sur la période plus récente. Ces différences étaient plus marquées chez les neurologues pratiquant une activité en centre expert ou en hospitalier par rapport aux neurologues exerçant une activité libérale. Conclusion : La prescription d’un traitement de fond dans la SEP s’est modifiée entre 2007 et 2022 avec un délai avant le premier traitement plus court, une proportion plus élevée de patients recevant d’emblée un traitement de fond de haute efficacité et un profil de ces patients modifié. Ces changements étaient plus marqués concernant les prescriptions réalisées par les neurologues du centre expert et en hospitalier par rapport aux neurologues libéraux., Background: The evolution of the therapeutic arsenal, diagnostic criteria and recommendations concerning treatment in multiple sclerosis (MS) have led to a change in the practices of neurologists related to the prescription of disease-modifying treatment (DMT) in this disease. Our study focuses on the evolution of the prescriptions of a disease- modifying treatment in relapsing-remitting (RR) MS by neurologists with a private practice, hospital or MS center activity, between 2007 (introduction of natalizumab) and 2022 (current practice). Methods: We conducted a multicenter, descriptive retrospective study using the EDMUS database, focusing on the prescription of DMTs for patients with RRMS in the Alsace region. The study encompassed prescriptions initiated between January 2007 and July 2022, for patients who had undergone a brain MRI within 6 months of disease onset. The DMTs considered as high efficacy included fingolimod, cladribine, rituximab, ocrelizumab, ofatumumab, and natalizumab. Main results: In our study, focused on the initial prescription of DMTs for multiple sclerosis (MS) involving a cohort of 1218 patients, we observed a significant time reduction before the first DMT was prescribed, from approximately 1 year during the 2007-2010 period to less than 4 months in the more recent 2018-2022 timeframe. The proportion of high-efficacy treatments (HET) increased notably during the later period interval compared to the earlier ones and there was a discernible change in the profiles of patients initiating HET. In fact, patients were older and exhibited less clinical severity in the recent timeframe.These disparities in treatment and patient profiles were particularly pronounced among neurologists practicing within specialized centers or hospitals, in contrast to those engaged in private practices. Conclusions: The prescription of a DMT in RRMS changed between 2007 and 2022 with a faster prescription of the first DMT, a higher proportion of patients receiving HET from the onset and a shift in the profile of these patients. These changes were more marked for the prescriptions made by the neurologists of an MS center and in hospital compared to private neurologists., Thèses et écrits académiques
