Une fois sorti du service de greffe, le patient pédiatrique et son aidant, en général l'un des parents, ont la responsabilité des prises médicamenteuses. Expert du médicament, le pharmacien peut réaliser des entretiens pharmaceutiques à différents moments de la prise en charge pour sécuriser le retour à domicile et favoriser l'adhésion thérapeutique du patient. Le travail réalisé aux Hôpitaux Universitaires de Strasbourg sur une période d'un an a consisté en la mise en place d'entretiens pharmaceutiques à destination de cette population de patients à deux moments clés de la prise en charge : lors de la préparation du patient à la sortie d'hospitalisation et en phase de suivi post-greffe. Allogeneic hematopoietic stem cell transplantation is a proven treatment for hematological malignancies and non-malignant diseases in pediatric medicine. It requires the patient to be hospitalized for several weeks for the transplant procedure, followed by several months of follow-up and daily medication. Patient prognosis and adherence to medication are closely linked. The work carried out at the University Hospitals of Strasbourg over a one-year period involved setting up pharmaceutical consultations for this patient population at two key points in their care : when preparing patients for discharge from hospital, and during the post-transplant follow-up phase. A complete pharmaceutical analysis of the prescription, pharmaceutical interventions with the medical team and families, and the provision of personalized information aids have helped to optimize therapeutic management.
