Description : Médecine (Endocrinologie, diabétologie et nutrition), Cette étude rétrospective se base sur les données cliniques, biologiques et radiologiques d'une cohorte de 33 patients suivis aux Hôpitaux Universitaires de Strasbourg et à l'Institut de Cancérologie de Strasbourg (ICANS), traités par immunothérapies pour une pathologie cancéreuse. Les signes cliniques d'hypophysite sont peu spécifiques ce qui rend son constat difficile, souvent responsable d'un retard diagnostic. L'insuffisance corticotrope est omniprésente au diagnostic d'hypophysite, profonde et majoritairement irréversible. Les insuffisances thyréotrope et gonadotrope sont fréquemment retrouvées au diagnostic et récupèrent généralement dans les premiers mois. Le déficit gonadotrope est principalement retrouvé en cas de combinaison d'immunothérapies (anticorps anti-PD-L1/PD1 et anticorps anti-CTLA-4). Le délai médian d'apparition de l'hypophysite apparaît deux fois plus court en cas de combinaison de traitement comparativement aux patients traités par anticorps anti-PD-L1/PD1 seuls. L'aspect « ballonisé » de l'hypophyse, signature de l'hypophysite, n'est mis en évidence principalement que dans les 40 premiers jours de l'hypophysite, pouvant donc manquer en cas de retard à la réalisation de l'IRM hypophysaire. Nous avons observé un retard diagnostic d'hypophysite de plus de 9 semaines dans notre cohorte par rapport aux données de la littérature, ce retard diagnostic étant responsable d'un nombre important d'hospitalisations avec un grade de toxicité de l'hypophysite supérieur ou égal à 3 chez près de 43% des patients., This retrospective study is based on clinical, biological and radiological data from a cohort of 33 patients followed at the Strasbourg University Hospitals and the Strasbourg Cancer Institute (ICANS), treated with immunotherapies for a cancer pathology. The clinical signs of hypophysitis are not very specific, which makes it difficult to detect, often responsible for a delay in diagnosis. Corticotropic insufficiency is omnipresent at the diagnosis of hypophysitis, profound and mostly irreversible. Thyrotropic and gonadotropic insufficiencies are frequently found at diagnosis and generally recover within the first months. Gonadotropic deficiency is mainly found in the case of a combination of immunotherapies (anti-PD-L1/PD1 antibodies and anti-CTLA-4 antibodies). The median time to onset of hypophysitis appears twice as short in the case of combination treatment compared to patients treated with anti-PD-L1/PD1 antibodies alone. The “ballooned” appearance of the pituitary gland, a signature of hypophysitis, is mainly only highlighted in the first 40 days of hypophysitis, and may therefore be missed in the event of a delay in performing the pituitary MRI. We observed a delay in diagnosis of hypophysitis of more than 9 weeks in our cohort compared to data in the literature, this diagnostic delay being responsible for a significant number of hospitalizations with a grade of hypophysitis toxicity greater than or equal to 3 in almost 43% of patients., Thèses et écrits académiques
