Description : Médecine (médecine générale), Introduction : L’objectif de cette étude est de déterminer si le dispositif PRADO-IN-CADO qui allie outils connectés, plateforme de traitement d’alertes par intelligence artificielle, et suivi médical ambulatoire au plus proche du patient améliore la prise en charge de l’insuffisance cardiaque (IC) chronique. Matériel et méthode : Il s’agit d’une étude mono-centrique bas-rhinoise, prospective de type non interventionnelle, contrôlée, non randomisée, et ouverte de cohorte incluant 216 patients insuffisants cardiaques répartis en deux groupes appariés : le groupe TS bénéficiant du dispositif PRADO-IC couplé à la télésurveillance (N=72), et le groupe P bénéficiant du programme PRADO-IC sans télésurveillance (N=144). Les patients du groupe TS se voient remettre à domicile en sortie d'hôpital une mallette comprenant une tablette tactile et des capteurs (pèse-personne, tensiomètre, et un oxymètre de pouls) communiquant automatiquement vers la plateforme MyPredi. L’IDE est informée en cas d’alerte via la plateforme, et en réfère au médecin traitant ou au cardiologue pour déterminer la prise en charge. Le critère de jugement principal est le taux de ré-hospitalisation pour décompensation cardiaque à 6 mois et à 1 an de l’inclusion. Les critères de jugements secondaires sont les taux de réhospitalisation toutes causes à 1 an, la mortalité cardiologique et globale, le nombre de jours d’hospitalisation pour IC aiguë et toutes causes. Une analyse multivariée a été réalisée intégrant âge, DFG, IMC, dyslipidémie et appartenance au groupe TS. Résultats : Le nombre moyen d’hospitalisations pour IC aiguë sur la période de 6 mois est 0.5 [0.3 – 0.7] pour le groupe TS vs. 0.45 [0.3 – 0.6] pour le groupe P, avec p = 0.7. Le nombre moyen d’hospitalisations pour IC aiguë sur 1 an est 0.9 [0.6 - 1.2] pour le groupe TS vs. 0.65 [0.5 - 0.8] pour le groupe P, avec p = 0.1. Les résultats sont également non significatifs pour les critères de jugements secondaires. Cependant, l’analyse inférentielle multivariée retrouve un impact négatif du DFG sur le critère de jugement principal. Conclusion : Les résultats non-significatifs et l’impact significatif du DFG sur le taux de ré-hospitalisation pour décompensation cardiaque à 6 mois et à 12 mois de l’inclusion ne permettent pas de conclure, à ce jour, à une amélioration de la prise en charge de l’IC par le dispositif PRADO-INCADO., Introduction : The objective of this study is to determine whether the PRADO-INCADO device, which combines connected tools, an alert processing platform using artificial intelligence, and ambulatory medical monitoring as close as possible to the patient, improves HF patient care. Materiel and methode: This is a single-center study in the BAS-RHIN region, prospective, non-interventional, controlled, non-randomized, and open cohort study including 216 heart failure patients divided into two matched groups: the TS group. benefiting from the PRADO-IC system coupled with remote monitoring (N=72), and the P group benefiting from the PRADO-IC program without remote monitoring (N=144). Patients in the TS group are given a briefcase at home after leaving the hospital including a toutchscreen tablet and sensors (scales, blood pressure monitor, and a pulse oximeter) that communicate automatically with the MyPredi platform. The nurse is informed in the event of an alert via the platform, and refers it to the GP or cardiologist to determine treatment. The main outcome is the re-hospitalization rate for acute HF at 6 months and 1 year. Secondary outcomes are all-cause re-hospitalization rates at 1 year, cardiological and overall mortality rates, average number of days for acute HF-hospitalizations and all-cause hospitalizations. A multivariate analysis was performed integrating age, GFR, BMI, dyslipidemia and belonging to the TS group. RESULTS: The average number of hospitalizations for acute HF over the 6-month period is 0.5 [0.3-0.7] for the TS group vs. 0.45 [0.3-0.6] for P group, with p=0.7. The average number of hospitalizations for acute HF over 1 year is 0.9 [0.6-1.2] for the TS group vs. 0.65 [0.5-0.8] for P group, with p=0.1. The results are also non-significant for the secondary outcomes. However, the multivariate analysis found a negative impact of GFR on the primary outcome. Conclusion : The non-significant results and the significant impact of GFR on the re-hospitalization rate for acute HF at 6 months and 12 months do not allow us to conclude of an improvement the HF Management by the PRADO-INCADO device., Thèses et écrits académiques
