Description : Médecine (oncologie), Introduction : Le cancer de la tête et cou est le 6ème cancer le plus fréquent dans le monde avec un pronostic défavorable dès le stade localement avancé. Le traitement standard chez les patients inopérables est la chimioradiothérapie exclusive mais certains patients ne peuvent pas bénéficier de ce traitement d’emblée. Nous nous sommes donc intéressés à une chimiothérapie d’induction par PCC avant traitement de clôture chez ces patients. Méthodes : Nous avons réalisé une étude rétrospective d’une cohorte de patients de deux hôpitaux d’Alsace, traités entre 2013 et 2022 avec une chimiothérapie d’induction par PCC pour un carcinome épidermoïde de la tête et du cou localement avancé inéligibles à une chirurgie ou CRT d’emblée pour des raisons d’extension tumorale, d’évolutivité ou de symptômes associés. Résultats : 119 patients ont été inclus dans l’étude. Les données d’efficacité retrouvent un ORR à 73,9% avec 62,2% de patients en réponse complète à la fin des traitements de clôture. La survie globale à 3 ans était de 46,6% (IC à 95% [0,356, 0,611]). Il y avait une amélioration significative des symptômes après chimiothérapie d’induction (dysphonie, dysphagie et douleurs) avec un profil de toxicité favorable. Conclusion : Cette étude donne un signal intéressant quant à l’efficacité et à la tolérance de la chimiothérapie d’induction par PCC dans le cancer épidermoïde de la tête et du cou localement avancé. Il s’agit de la plus grande cohorte, à ce jour, étudiant cette problématique., Introduction : Head and neck cancer is the 6th most common cancer worldwide, with a poor prognosis including the locally advanced stage. The standard treatment for inoperable patients is exclusive chemoradiotherapy (CRT), but some patients cannot benefit from this treatment due to tumor extension, progression or associated symptoms.We therefore investigated the interest of induction chemotherapy with PCC prior to definitive treatment in these patients. Methods : We conducted a retrospective study of a cohort of patients from two hospitals in Alsace, treated between 2013 and 2022 with Paclitaxel, Carboplatine and Cetuximab induction chemotherapy for a locally advanced squamous cell carcinoma of the head and neck ineligible for primary surgery or CRT. Results : A total of 119 patients were included in the study. Overall three-year survival was 46.6% (IC à 95% [0,356, 0,611]). Efficacy data showed 73.9% ORR after ICT, with 62.2% of patients in complete response at the end of definitive treatment. There was a significant improvement in symptoms after treatment (dysphonia, dysphagia, and pain), with a favorable toxicity profile. Conclusions : This study provides an interesting indication of the efficacy and tolerability of induction chemotherapy with PCC in locally advanced squamous cell carcinoma of the head and neck.
