Description : Médecine (médecine d'urgence), Le service des urgences est au cœur du système de soins Français. En 2022, 20 millions de patients ont été pris en charge aux urgences, et 5% présentait une douleur thoracique. Dans ce contexte, la complexité du travail de l'urgentiste est d'éliminer les pathologies graves nécessitant une prise en charge immédiate, notamment le syndrome coronarien aigu. L'objectif de cette étude est d'évaluer la performance diagnostique du scanner thoracique par la mise en évidence de calcifications coronaires pour infirmer la présence d'évènement coronarien aigu chez les patients se présentant pour une douleur thoracique. Il s'agit d'une étude rétrospective monocentrique, réalisée à Strasbourg ayant inclus tous les patients de plus de 18 ans consultant pour une douleur thoracique aux urgences, entre le 1er juin 2021 et le 31 décembre 2021 avec un électrocardiogramme non modifié et un premier dosage de troponine négatif. Le critère de jugement principal est la performance diagnostic des calcifications coronariennes dans l'élimination du syndrome coronarien aigu. Les critères de jugements secondaires sont la sensibilité et spécificité des calcifications dans le syndrome coronarien aigu, la comparaison avec la performance diagnostique de deux dosages de troponines, le taux de reconsultation , de rehospitalisation et d investigations dans les 2 mois suivant la consultation aux urgences. Parmi les 280 patients inclus, 141 ne présentaient pas de calcification et 14 évènements ont été retrouvés avec une valeur prédictive négative des calcifications de 99.8%. La sensibilité et la spécificité étaient respectivement de 98,4% et 53%. Parmi les patients du groupe sans calcification, 8,2% ont été hospitalisés et aucun n'a présenté d'événement coronarien aigu. Parmi les 10 patients (7%) de ce groupe qui ont consulté dans les 2 mois, 4 investigations ont été réalisées et étaient toute négative. Notre étude en vie réelle a permis de montrer la bonne performance diagnostique de la recherche de calcifications coronariennes sur le scanner thoracique chez les patients présentant un électrocardiogramme non modifié avec un dosage unique de la troponine pour'éliminer un syndrome coronarien aigu. Ces résultats apportent des perspectives intéressantes pour faciliter la stratification du risque d'évènement coronarien aigu aux urgences afin d’optimiser la filière de soin de ces patients qui représentent une part importante de l'activité., In 2022, 20 million patients were treated in emergency departments (Eds) in France, and 5% of them had chest pain. In this context, the complexity of the emergency physician is to eliminate serious pathologies requiring immediate treatment, in particular acute coronary syndrome. The aim of this study was to assess the diagnostic performance of chest CT in detecting coronary calcifications to rule out the presence of an acute coronary event in patients presenting with chest pain. This was a retrospective, single-center study carried out in an ED in France and included all patients over 18 years of age presenting with chest pain between 1 June 2021 and 31 December 2021 with an unaltered electrocardiogram and a negative first troponin assay. The primary endpoint is the diagnostic performance of coronary calcifications in ruling out acute coronary syndrome. The secondary endpoints are the sensitivity and specificity of calcifications in acute coronary syndrome, comparison with the diagnostic performance of troponins and the rate of reconsultation, rehospitalisation and investigations within 2 months of the ED. Of the 280 patients included, 141 didnt have calcifications. Among 14 events were found with a negative predictive value for calcifications of 99.8%. Sensitivity and specificity were 98.4% and 53% respectively. 8.2% of the non-calcification group were admitted to hospital and none suffered an acute coronary event. 36 patients (12.8%) consulted a doctor within 2 months, with 23 investigations, all of which were negative in the non-calcification group. The use of coronary calcifications on chest CT in patients with an unaltered electrocardiogram with a single troponin assay appears to be a potential tool in risk stratification for acute coronary events. The results are in line with previous studies and provide future perspectives. They encourage a larger-scale study to assess the medico-economic implications of this strategy., Thèses et écrits académiques
