Description : Médecine (chirurgie pédiatrique), Introduction : Les malformations broncho-pulmonaires (MBP) sont un groupe hétérogène de malformations congénitales des poumons incluant les Malformations adénomatoïdes kystiques (MAK) et les séquestrations pulmonaires (SP). Les morbidités au moyen et long cours des enfants atteints et opérés sont peu étudiées dans la littérature actuelle. De plus, les recommandations pour la surveillance de ces enfants ne sont pas consensuelles. Objectif : Étudier les morbidités respiratoires, musculo-squelettiques et esthétiques à moyen et long terme des enfants atteints et opérés de MAK et de SP et analyser le suivi de ces enfants en extra-hospitalier. Méthodes : Cette étude rétrospective monocentrique a inclus tous les patients atteints et opérés de MAK et de SP au CHRU de Strasbourg entre 2005 à 2022. Les données anténatales, néonatales, cliniques, radiologiques, opératoires et post-opératoires ont été colligés. Les données sur les morbidités et leurs répercussions sur la vie quotidienne ont été recueillies par questionnaire dédié chez les enfants âgés de plus de 4 ans. Les données concernant la surveillance en ville ont été recueillies par questionnaire auprès des médecins et/ou pédiatres traitant inscrit dans le dossier patient. Résultats : 66 enfants ont été pris en charge pour MAK et/ou de SP dans le service de chirurgie pédiatrique du CHU de Strasbourg, dont 2 enfants présentaient une forme bilatérale. Environ 83 % des enfants ont eu un diagnostic anténatal de leur lésion. 36 % des fœtus présentaient de signes de gravité dont 1,3 ont bénéficié d’un geste anténatal. Tous les enfants ont été opérés à un âge médian de 8 mois permettant de réséquer 67 lésions. Environ 86 % des enfants ont pu être opérés par voie mini-invasive, une lobectomie a été effectuée dans 35% des résections. Nous avons répertorié environ 24 % de complications post-opératoire dans le mois suivant la chirurgie principalement des bullages prolongés. Une seule consultation de suivi a été effectuée chez la moitié de notre cohorte. Lors de ce suivi, un résidu de la malformation initiale a été retrouvé chez 6 enfants, dont 2 ont nécessité une chirurgie complémentaire. Au cours du suivi à moyen et long terme, nous avons analysé 16 dossiers soit seulement 24% de notre population totale. Parmi les enfants répondeurs, 81 % présentaient une morbidité : 50 % des enfants avaient une morbidité respiratoire (infections broncho-pulmonaire, pneumothorax, réactivité bronchique). Environ 31 % d’enfants présentaient une morbidité musculo-squelettique (déformation de la paroi thoracique et/ou scoliose). Enfin, 44 % des enfants étaient moyennement satisfaisait de leur cicatrice thoracique dont 29 % ont émis le souhait d’une reprise chirurgicale de leur cicatrice. Nous avons pu voir que 88 % de ces enfants se considèrent en excellent ou bon état de santé générale. Enfin, nous avons récolté 7 réponses de professionnels de santé de ville (10 % de réponse). Les données concernant la surveillance clinique, la réalisation d’examens complémentaires radiologique et la réalisation d’explorations fonctionnelles respiratoires étaient hétérogènes. Cependant, la quasi-totalité des professionnelles de ville ayant répondu a trouvé un réel intérêt vis-à-vis d’un suivi prolongé au CHRU en collaboration avec la médecine de ville. De plus, la totalité des praticiens sont en faveur de la création d’une notice d’informations sur la pathologie et sur le suivi adapté pour ces enfants. Conclusion : Dans notre étude, les enfants atteints de MAK et/ou SP, ayant répondu au questionnaire, semblent présenter une proportion non-négligeable de morbidités à moyen et long terme. Cependant, il semblerait que le retentissement soit faible et n’entrave pas la vie quotidienne de ces enfants. Dans le but d’améliorer la collaboration hôpital-ville, il semblerait intéressant d’organiser des consultations de suivi multidisciplinaire au CHU régulière jusqu’à l’âge adulte et de délivrer une notice d’information sur la pathologie et le suivi adapté chez ces enfants. , Introduction : Congenital lung malformations(CLM) are a heterogeneous group of congenital malformations of the lungs, including congenital pulmonary airway malformations(CPAM) and pulmonary sequestrations(PS). Results : Sixty-six children were treated for CPAM and/or PS in the pediatric surgery department of Strasbourg University Hospital, including 2 children with a bilateral form. Approximately 83% of children had an antenatal diagnosis of their lesion. 36% of fetuses showed signs of severity, of which around 33% benefited from an antenatal procedure. All children underwent surgery at a median age of 8 months, enabling 67 lesions to be resected. Some 86% of children underwent minimally invasive surgery, with lobectomy performed in 35% of resections. We recorded around 24% of post-operative complications in the month following surgery, mainly prolonged bubbling. A single follow-up visit was carried out in half of our cohort. During this follow-up, a residue of the initial malformation was found in 6 children, 2 of whom required additional surgery. During medium- and long-term follow-up, we analyzed 16 cases, representing only 24% of our total population. Among responders, 81% had morbidity: 50% of children had respiratory morbidity (bronchopulmonary infections, pneumothorax, bronchial reactivity). Some 31% of children had musculoskeletal morbidity (chest wall deformity and/or scoliosis). Finally, 44% of the children were moderately satisfied with their thoracic scars, with 29% expressing the wish to have their scars surgically re-done. We found that 88% of these children considered themselves to be in excellent or good general health. Finally, we collected 7 responses from healthcare professionals (10% response rate). Data on clinical surveillance, radiological examinations and respiratory function tests were heterogeneous. However, almost all of the healthcare professionals who responded found a real interest in prolonged follow-up at the CHRU in collaboration with outpatient medicine. What's more, all practitioners were in favor of creating an information leaflet on the pathology and appropriate follow-up for these children. Conclusion : In our study, children with CPAM and/or PS who responded to the questionnaire appear to have a non-negligible proportion of medium- and long-term morbidity. However, it would appear that the repercussions are minor and do not interfere with these children's daily lives. In order to improve hospital-city collaboration, it would seem worthwhile to organize regular multidisciplinary follow-up consultations at the CHU until adulthood, and to issue an information leaflet on the pathology and appropriate follow-up for these children., Thèses et écrits académiques
