<dc:description xsi:type="unistra:Resume" xml:langue="fre">Depuis une trentaine d’années, l’implantologie orale connaît un réel essor avec l’apparition du principe d’ostéo-intégration du Professeur Bränemark. De nombreux progrès ont été faits dans ce domaine et il s’agit d’un traitement éprouvé. Les chirurgiens-dentistes sont amenés à poser de plus en plus d’implants dans le cadre de la réhabilitation prothétique des édentements, mais il convient de toujours évaluer le rapport bénéfice/risque et de l’exposer au patient. En effet, ce dernier doit être conscient que tout acte chirurgical présente un risque de complications, et même d’échecs, qui imposent parfois le retrait de l’implant. Outre les risques liés à l’acte chirurgical, des complications post-opératoires ou tardives peuvent survenir. Quels sont les différents types de complications auxquels le praticien peut être confronté ? Quand parle-t-on d’échec implantaire? Quels sont les moyens de gérer les échecs implantaires? Dans une première partie ce travail fera à la fois l’analyse des raisons qui poussent à devoir déposer l’implant : techniques, mécaniques, biologiques, ou esthétiques et fonctionnelles, et l’analyse des facteurs de risques diminuant la pérennité de l’implant. Dans un second temps il s’agira de décrire les différentes techniques qui s’offrent à nous pour effectuer la dépose des implants selon qu’ils soient ostéo-intégrés ou non-ostéo-intégrés. Aujourd’hui l’amélioration des techniques chirurgicales permet au praticien d’être de moins en moins invasif et de garantir une réhabilitation optimale. Enfin, dans un troisième temps, nous discuterons au moyen de cas cliniques des perspectives de traitement proposées après l’explantation</dc:description>
<dc:description xsi:type="unistra:Resume" xml:langue="fre">Since 30 years, oral implantology has a real boom with the emergence of the principle of osteo-integration of Professor Bränemark. Many progress has been made in this area and it is a proven treatment. The dentists have to place more and more implants to the prosthetic rehabilitation of the edentulous space, but should always assess the risk-benefit ratio and expose it to the patient. Indeed, the latter should be aware that any surgery poses a risk of complications, and even failures, which sometimes require removal of the implant. Also the risks associated with the surgery, post-operative or late complications may occur. What are the different types of complications to which the practitioner may be confronted? When spoken of implant failure? What are the means to manage the implant failures? In the first part this work will both make the analysis of the reasons which push to remove the implant: technical, mechanical, biological, or aesthetic and functional, and the analysis of the risk factors decreasing the longevity of the implant. In a second step it will be to describe the different techniques that are available to us to perform the removal of implants depending on whether they are osteo-integrated or non-osteo-integrated. Today the improvement of surgical techniques allows the practitioner to be less invasive and ensure optimal rehabilitation. Finally, in a third time, we will discuss using clinical cases of the treatment perspectives proposed after explantation</dc:description>
