<dc:description xml:langue="" xsi:type="unistra:Resume">La maladie veineuse thromboembolique (MVTE) constitue un problème majeur de santé publique. Elle comprend les thromboses veineuses superficielles et profondes, ainsi que leur complication la plus grave : l’embolie pulmonaire.C’est une maladie multifactorielle et elle ne présente pas de signe clinique pathognomonique. Les connaissances sur sa physiopathologie ont évolué au cours des dernières années, avec notamment : la réorganisation de la cascade de coagulation en réseau de coagulation et la découverte du processus d’immunothrombose. L’utilisation des anticoagulants oraux directs est aujourd’hui suggérée en 1ère intention dans le traitement de la MVTE, par rapport aux antagonistes de la vitamine K. Pour assurer sa bonne prise en charge de nombreux professionnels de santé interviennent, dont le pharmacien d’officine, qui occupe une place de choix dans ce parcours de soins. Leur coopération et la mise à disposition d’outils de formation et d’information pour ces professionnels sont indispensables.</dc:description>
<dc:description xml:langue="" xsi:type="unistra:Resume">Venous thromboembolism disease (VTE) is a major public health issue. Theres is two mains from of the disease : superficial venous thrombosis and deep vein thrombosis, as well as their major complication : pulmonary embolism. VTE is a multifactorial disease and that doesn’t show any pathognomonic clinical sign. The knowledge about his pathophysiology has evolved during the last years, especially with the reorganization of the coagulation cascade into a coagulation network and the discovery of the process of immunothrombosis. The use of direct oral anticoagulants is today recommended on first intention on the VTE treatment, compared to the vitamin K antagonists. The care of this disease involves many health professionals, including the pharmacist, who has a place of choice in the healthcare pathway. To provide the good practice for the management, a cooperation of all health professionals concerned and the provision of training and information tools for this health professionals are essential.</dc:description>
