Prévention de la survenue d’ostéoradionécrose en chirurgie orale : nouvelles techniques de radiothérapie et protocole d’avulsion dentaire : thèse présentée pour le diplôme d'État de docteur en chirurgie dentaire le 5 décembre 2025

Résumé(s) : Les cancers des voies aérodigestives supérieurs représentent 10% des cancers en France et la radiothérapie fait partie intégrante de l’arsenal thérapeutique pour lutter contre ces types de cancer. Nous sommes de plus en plus confrontés, dans notre exercice, à des patients qui nécessitent une recherche de foyers infectieux bucco-dentaire en prévision d’une radiothérapie ainsi qu’à des patients ayant déjà subi ce type de protocole et nécessitant une ou des avulsions dentaires pouvant donner lieu à une complication rare mais sévère : l’ostéoradionécrose. Des avancées tant en chirurgie orale qu’en radiothérapie ont modifié la prise en charge de ces patients au niveau bucco-dentaire dans le but de diminuer les risques d’ostéoradionécrose, complication grave pouvant entrainer un déficit fonctionnel et esthétique irréversible. Dans une première partie ; nous détaillerons les risques fréquemment rencontrés lors d’avulsion(s) dentaire(s) qui peuvent, par ailleurs, majorer la possibilité de survenue de cette complication. Dans une deuxième partie, nous détaillerons les différents protocoles de radiothérapie ainsi que les innovations récentes dans ce domaine ayant pour but un ciblage plus précis des cellules cancéreuses et permettant une moindre irradiation des tissus sains environnants.Enfin, nous apporterons une proposition de protocole d’avulsions dentaire en fonction du type de radiothérapie et de la dose afin d’avoir un risque de survenue d’ostéoradionécrose le plus faible possible, Cancers of the upper aerodigestive tract represent 10% of cancers in France, and radiotherapy is an integral part of the therapeutic arsenal used to combat these types of cancer. We are increasingly confronted, in our practice, with patients who require a search for oral infectious foci in anticipation of radiotherapy, as well as with patients who have already undergone this type of protocol and who require one or more dental extractions that may give rise to a rare but severe complication: osteoradionecrosis. Advances both in oral surgery and in radiotherapy have modified the management of these patients at the oral level with the aim of reducing the risks of osteoradionecrosis, a serious complication that can lead to irreversible functional and aesthetic impairment. In a first part, we will detail the risks frequently encountered during dental extraction(s) which may, moreover, increase the possibility of the occurrence of this complication. In a second part, we will detail the different radiotherapy protocols as well as the recent innovations in this field aimed at a more precise targeting of cancer cells and allowing for less irradiation of the surrounding healthy tissues. Finally, we will provide a proposed dental extraction protocol depending on the type of radiotherapy and the dose, in order to achieve the lowest possible risk of osteoradionecrosis
