<dc:description xml:langue="" xsi:type="unistra:Resume">Cette thèse s'appuie sur trois Récits de Situations Complexes Authentiques (RSCA) rencontrés au cours de l'internat de médecine générale. Le premier se réfère à une patiente allophone, chez qui la barrière linguistique et l'absence d'interprétariat professionnel ont mis à mal le raisonnement médical, menant à un retard de prise en charge. Le deuxième revient sur une patiente souffrant de douleurs chroniques, où la réticence pharmacologique, la dépression masquée et la nécessité d'un suivi structuré ont mis en évidence l'apport décisif de l'infirmier ASALEE et les limites d'un soin isolé. Le troisième concerne un enfant fébrile dont la trajectoire a été modifiée par la subjectivité de la clinique, la pression parentale, la dynamique hiérarchique et des enjeux médico-légaux, illustrant le risque de surmédicalisation et l'importance d'une cohérence d'équipe. Ces situations mettent en évidence trois dimensions transversales de la médecine générale : l'incertitude comme donnée constitutive du soin ; la relation médecin-patient (et entourage) comme déterminant du raisonnement et de l'adhésion ; l'organisation collective comme condition de qualité et de sécurité. Elles soulignent que bien soigner ne revient pas à viser la certitude, mais à construire, avec le patient et l'équipe, une trajectoire claire, ajustée et partagée. Le RSCA m'apparaît ainsi comme un outil de formation fécond, transformant l'expérience singulière en enseignement transmissible et nourrissant un apprentissage continu, réflexif et coordonné</dc:description>
<dc:description xml:langue="" xsi:type="unistra:Resume">This thesis is based on three Authentic Complex Situation Reports (ACSR) encountered during general medicine residency. The first refers to an allophone patient, in whom the language barrier and the absence of professional interpretation undermined medical reasoning, leading to a delay in treatment. The second recounts a patient suffering from chronic pain, where pharmacological reluctance, masked depression, and the need for structured follow-up highlighted the decisive contribution of the ASALEE nurse and the limitations of isolated care. The third concerns a child with a fever whose trajectory was altered by the subjectivity of the clinic, parental pressure, hierarchical dynamics, and medico-legal issues, illustrating the risk of over-medicalization and the importance of team consistency. These situations highlight three cross-cutting dimensions of general medicine: uncertainty as a fundamental aspect of care; the doctor-patient (and family) relationship as a determinant of reasoning and adherence; and collective organization as a condition for quality and safety. They emphasize that good care is not about seeking certainty, but about building, together with the patient and the team, a clear, adjusted, and shared trajectory. I therefore see RSCA as a fruitful training tool, transforming unique experiences into transferable lessons and fostering continuous, reflective, and coordinated learning</dc:description>
