Description : Médecine. Chirurgie dentaire, Résumé : Les implants dentaires constituent aujourd’hui la solution de choix pour la réhabilitation prothétique. Toute réhabilitation par prothèse sur implants doit être précédée d’un bilan pré-implantaire pour l’élaboration d’un projet prothétique. Cette intervention peut être réalisée à l’aide d’un guide chirurgical fabriqué soit de manière artisanale soit par ordinateur : c'est la planification assistée par informatique. Dans un premier temps nous abordons la chirurgie guidée, sa définition, et sa précision. Nous comparons dans un second temps les quatre principaux systèmes de chirurgie guidée: Simplant®, NobelClinician®, Easyguide® et Skyplanx® illustré d’un cas clinique. Dans un troisième temps nous nous penchons sur la radiographie, première étape de la confection d’un guide chirurgical. Nous observons les artéfacts lors de la réalisation du CBCT, les artefacts cinétiques, mécaniques, ou de durcissements et les problèmes liés à la segmentation. Une étude prospective de 20 patients a été réalisée sur les artéfacts de mouvements par le calcul des écarts entre les scans de modèles d’études et les CBCT des patients. Les patients ayant une arcade complète mandibulaire, sans lésions dentaires ni réhabilitations prothétiques de 35 à 45 ont été choisis. Trois points de repères ont été fixés: le sommet des cuspides vestibulaires des premières prémolaires et le milieu interincisif.Après la superposition du CBCT du patient et du scan du modèle d’étude, au niveau de coupe coronale des canines, la distance la plus élevée entre la pointe canine du scan et celle du modèle a été mesurée. Pour conclure, s‘il y a une différence entre le scan et le CBCT, cela correspond en partie au mouvement du patient. Enfin les complications qui peuvent découler de la chirurgie guidée et ces limites., Summary : Today, implants are a central solution of total or partiel edentation. A preclinical diagnosis should be studied before all prothesical rehabilitation with implant. A surgical guide can help this intervention. It can be realised in a craft way or by computer: it's the Computer-assisted planning. First, we talk about guided surgery, its definition, and its precision. We compare, in a second time, the four main software systems of guided surgery: Simplant, NobelClinician, EasyGuide and Skyplanx (with clinics cases). We discuss the radiography, first step of surgical guide making. And we see artifacts during CBCT realisation like motion and kinetics artifacts, mecanics artifacts, beam hardening and problem of image processing as segmentation. A prospective study of 20 patients on the motions artifacts was realized by calculating the gap between the scan of the study model in plaster and the CBCT of the patient. Patients having a complete arch, without dental hurts nor prothetic rehabilitations, were choosen. Three landmarks were fixed: the vestibular cusps of second premolars summit and the interincisive middle. We let stack the patient CBCT and the study model scan, and at the level of a sagittal cup of canine cusp point (said corronale cup) we estimate the highest distance between the canine headland of the CBCT and that of the model scan. We demonstrate while if there is a difference between the scan and the CBCT, it corresponds partially to the movement of the patient. Finally we finish by the complications and the guided surgery limits.
