Incidence et intérêt du dosage de la lactatémie capillaire en salle de surveillance post-interventionnelle

Anesthésie-réanimation, INTRODUCTION : La période post-opératoire immédiate est une période à risque qui nécessite une surveillance étroite des patients en salle de surveillance post interventionnelle (SSPI). Le monitorage en SSPI est essentiellement clinique et il n’existe aucune recommandation imposant la réalisation d’examens complémentaires. L’intérêt diagnostique, pronostique et thérapeutique du dosage du lactate a largement été démontré dans la prise en charge du patient critique. Il a été démontré une parfaite corrélation de lactatémie capillaire par rapport à la lactatémie artérielle. L’objectif de notre étude est de déterminer l’incidence de l’hyperlactatémie capillaire(HLC) post opératoire immédiate et d’évaluer la valeur pronostique de l’HLC. MATERIEL ET METHODES : Nous avons réalisé une étude prospective observationnelle sur la période de décembre 2017 à avril 2018. Les patients inclus étaient des patients stables, opérés d’une chirurgie classée à risque cardiovasculaire intermédiaire ou majeur. Deux mesures de lactatémie capillaire digitale ont été réalisées, dès l’admission en SSPI(T0) et avant la sortie de SSPI(T1). Le critère de jugement(CJ) principal était la présence d’une lactatémie capillaire supérieure à 2,3 mmol/l. Le CJ secondaire était la recherche de complications post opératoires immédiates et jusqu’au 7eme jour. RESULTATS : 95 patients ont été inclus. L’incidence de l’hyperlactatémie capillaire en post opératoire était de 72,6%. La valeur moyenne de lactatémie était de 2,9 ± 2 mmol/L à T0 et de 3 ± 2,3 mmol/L à T1. Un saignement peropératoire important était considéré comme un facteur de risque d’HLC. Il n’y avait pas de différence concernant le devenir des patients en présence ou non d’une hyperlactatémie capillaire. CONCLUSION : Notre étude préliminaire met en évidence une incidence élevée d’HLC en post opératoire immédiat. L’absence de différence en terme de complications peut s’expliquer par un manque de puissance du fait de notre faible effectif. Il serait intéressant d’élargir notre effectif afin de préciser l’incidence et mettre en évidence que l’hyperlactatémie est associée à un moins bon pronostic., INTRODUCTION: The immediate postoperative period is a risk period that requires close monitoring of patients in the post-interventional surveillance room (IPS). Perfect correlation of capillary lactatemia with arterial lactatemia has been demonstrated. The aim of our study is to determine the incidence of immediate postoperative capillary hyperlactatemia (HLC) and to evaluate the prognostic value of HLC. MATERIALS AND METHODS: We carried out a prospective observational study over the period from December 2017 to April 2018. The patients included were stable patients, operated on a surgery classified as intermediate or major cardiovascular risk. Two measurements of digital capillary lactatemia were performed, upon admission to SSPI (T0) and before the release of SSPI (T1). The primary endpoint (CJ) was the presence of capillary lactate greater than 2.3 mmol / l. The secondary CJ was looking for immediate postoperative complications and until the 7th day. RESULTS: 95 patients were included. The incidence of capillary hyperlactatemia postoperatively was 72.6%. The mean value of lactatemia was 2.9 ± 2 mmol / L at T0 and 3 ± 2.3 mmol / L at T1. Intraoperative bleeding was considered a risk factor for HLC. There was no difference in the outcome of patients with or without capillary hyperlactatemia. CONCLUSION: Our preliminary study shows a high incidence of HLC in the immediate postoperative period. The lack of difference in terms of complications can be explained by a lack of power because of our small size. It would be interesting to expand our staff to clarify the incidence and highlight that hyperlactatemia is associated with a poorer prognosis.
