L'index de bactéricidie : tentative d’établissement d’un nouveau paramètre dans la prise en charge des infections à entérobactéries résistantes

Anesthésie réanimation, Résumé : la prise en charge des infections à bactérie multi-résistantes est un défi quotidien pour le réanimateur et la part des bactéries multi résistantes dans l’épidémiologie des infections en réanimation ne cesse de croître malgré l’arsenal mis en place pour y faire face avec à la clé un impact considérable sur la morbidité, la mortalité et les coûts. Le travail présenté ici s’inscrit dans cette démarche visant à trouver des solutions annexes à l’antibiothérapie seule afin de faire face aux infections à germes multi résistants en réanimation. Nous nous sommes interrogés sur la possibilité de prédire, via les moyens utilisés en routine en laboratoire que sont la Concentration Minimale Inhibitrice et les seuils de sensibilité déterminés par le European committee on Antimicrobial Susceptibility testing (EUCAST), la bactéricidie d’un couple antibiotique/entérobactérie multi résistante. Il est possible qu’à paramètres égaux (sensibilité, site de diffusion, toxicité, allergies du patient, volume de distribution…) il soit avantageux d’utiliser l’antibiotique le plus bactéricide in vitro parmi un choix (souvent restreint) d’antibiotiques actifs., Sumary : the management of multi-resistant bacterial infections is a daily challenge for the intensivist and the share of multi-resistant bacteria in the epidemiology of infections in intensive care continues to grow despite the arsenal set up to cope with it and has a considerable impact on morbidity, mortality and costs. The work presented here is part of this process aimed at finding adjunctive solutions to antibiotherapy alone in order to deal with multi-resistant infections in intensive care. We wondered about the possibility of predicting, through the means used in laboratory routine that are the Minimum Inhibitory Concentration and the sensitivity thresholds determined by the European Committee on Antimicrobial Susceptibility Testing (EUCAST), the bactericidal activity of the couple antibiotic/multi-resistant enterobacteria. It is possible that at equal parameters (sensitivity, diffusion site, toxicity, patient allergies, volume of distribution ...) it is advantageous to use the most bactericidal antibiotic in vitro among a (often restricted) choice of active antibiotics.
