Évaluation des performances diagnostiques et de la reproductibilité des paramètres métaboliques quantitatifs de la TEP-TDM a la 18F-FDOPA dans la prise en charge des patients avec tumeurs neuroendocrines pancréatiques

Description : Médecine nucléaire, Introduction : les tumeurs neuro-endocrines pancréatiques (TNEP) sont des tumeurs rares dont la prévalence a nettement augmenté ces dernières années. Lorsque les TNEP sont non fonctionnelles, leur diagnostic est souvent tardif car les symptômes n’apparaissent qu’au stade de tumeur compressive ou de métastases. Les méthodes d’imagerie métabolique comme la scintigraphie ou la TEP-TDM aux récepteurs de la somatostatine jouent un rôle clef dans le diagnostic et le suivi thérapeutique de ces patients. En revanche, les données concernant la TEP-TDM à la 18F-FDOPA dans cette indication sont limitées et sa réalisation reste optionnelle selon les dernières recommandations. Pour cette raison, nous avons évalué les performances diagnostiques de la TEP-TDM à la 18F-FDOPA avec prémédication par Carbidopa et acquisitions précoces dans le diagnostic des TNEP, ainsi que la reproductibilité des paramètres métaboliques quantitatifs de la tumeur. Matériels et Méthodes : Les performances diagnostiques (détection de la tumeur primitive et bilan d’extension locorégionale et à distance) de la TEP-TDM à la 18F-FDOPA, réalisée dans le cadre du bilan pré-chirurgical de 22 patients avec TNEP, ont été analysées de manière rétrospective par deux médecins nucléaires, et comparées à celles de l’imagerie conventionnelle et à la scintigraphie des récepteurs de la somatostatine. Les paramètres quantitatifs concernant la fixation tumorale de la 18F-FDOPA (SUVmax, SUVpic, SUVmean et Volume Tumoral Métabolique ou VTM) ont été mesurés et leur reproductibilité a été évaluée. Une comparaison de ces paramètres au taux de Chromogranine A, grade, TNM et Ki67 des tumeurs a été réalisée. Résultats : Pour la détection de la tumeur primitive, la TEP-TDM à la 18F-FDOPA présente une sensibilité de 95% au temps précoce et 82% lors des images standard. Parmi les 5 patients avec atteinte ganglionnaire histologiquement prouvée, 2 ont été identifiés à l’imagerie conventionnelle (sensibilité : 40%, spécificité : 88%) et 3 à la TEP-TDM à la 18F-FDOPA (sensibilité : 60%, spécificité : 94%). La TDM et la TEP-TDM ont permis d’identifier les deux patients avec une maladie métastatique hépatique. Les SUVmax, SUVpic, SUVmean et VTM de la tumeur se révèlent très reproductibles (ICC tous supérieurs à 0,87). Seule une corrélation entre le VTM et le taux de Chromogranine A a été retrouvée (R2=0,52). Conclusion : La TEP-TDM à la 18F-FDOPA avec prémédication par Carbidopa présente une sensibilité élevée dans la prise en charge diagnostique pré-chirurgicale des patients avec TNEP pour la détection de la tumeur primitive et pour le bilan d’extension loco-régionale et à distance. En l’absence de disponibilité de la TEP/TDM aux récepteurs de la somatostatine, la TEP-TDM à la 18F-FDOPA est une alternative diagnostique valide. De plus, les paramètres métaboliques quantitatifs testés lors de notre étude, comme les SUV et le VTM tumoraux, pourront bien se prêter à une utilisation clinique en routine pour l’évaluation de l’efficacité du traitement (analogues non marqués de la somatostatine, thérapie ciblée, etc.) lors du suivi longitudinal des patients avec TNEP., Introduction : pancreatic neuroendocrine tumors (PNET) are rare but their prevalence has significantly increased in the last decades. When PNET are non-functional, their diagnosis is often delayed because the symptoms are linked to the tumoral volume or to the metastases. Metabolic imaging methods such as somatostatin receptor scintigraphy or PET-CT play a key role in the diagnosis and therapeutic follow-up of these patients. On the other hand, data concerning 18F-FDOPA PET-CT in this indication are limited and its implementation remains optional according to the latest recommendations. For this reason, we evaluated the diagnostic performance of 18F-FDOPA PET-CT with Carbidopa premedication and early acquisition in the diagnosis of TNEPs, as well as the reproducibility of the quantitative metabolic parameters of the tumor. Materials and methods : The diagnostic performance of the 18F-FDOPA PET-CT, performed on the pre-surgical assessment of 22 patients with PNET, were retrospectively analyzed by two nuclear physicians, and compared to the performances of conventional imaging and somatostatin scintigraphy. Quantitative parameters for 18F-FDOPA tumor uptake (SUVmax, SUVpic, SUVmean and Metabolic Tumoral Volume or MTV) were measured and their reproducibility has been evaluated. A comparison between those parameters and the chromogranin A levels, TNM, tumoral grade and Ki67 levels was performed. Results: For detection of the primary tumor, 18F-FDOPA PET-CT has a sensitivity of 95% at the early acquisition and 82% at the standard images. Of the 5 patients with histologically proven lymph node involvement, 2 were identified with conventional imaging (sensitivity: 40%, specificity: 88%) and 3 with 18F-FDOPA PET-CT (sensitivity: 60%, specificity: 94%). CT and PET-CT identified the two patients with metastatic liver disease. The SUVmax, SUVpic, SUVmean and MTV of the tumor are very reproducible (ICC all higher than 0.87). Only a correlation between MTV and Chromogranin A level was found (R2 = 0.52). CONCLUSION: The 18F-FDOPA PET-CT with premedication by Carbidopa has a high sensitivity in the pre-surgical diagnostic management of patients with PNET for the detection of the primary tumor and for loco-regional extension assessment. In the absence of somatostatine receptors PET / CT, 18F-FDOPA PET-CT is a valid alternative. In addition, the quantitative metabolic parameters tested in our study, such as tumor SUVs and VTM, may well be used in clinical routine for evaluation of treatment efficacy (somatostatin analogues, etc.) during the follow-up of patients with PNET.
