<dc:description xsi:type="unistra:Resume" xml:langue="fre">Introduction : l’identification des facteurs de risque d’IVD après implantation d’une AVG est un enjeu majeur afin de prévenir cette complication. Objectif : l’objectif de cette étude était de déterminer les facteurs de risque associés à la survenue d’une IVD. Méthodes : 68 patients ont bénéficié une AVG. Le critère de jugement principal était la survenue d’une IVD. Résultats : une IVD après implantation d’une AVG a été observée chez 32 patients. En analyse univariée, l’élévation des TGO (P=0,028) et un score inotrope vasoactif préopératoires élevés étaient significativement associés à une augmentation du risque d’IVD alors que l’implantation de l’AVG par thoracotomie était associée à une réduction significative de ce risque. L’analyse multivariée a objectivé que seule l’approche par thoracotomie était significativement associée à une diminution du risque d’IVD. Conclusions : l’implantation d’une AVG par thoracotomie permet de réduire significativement l’incidence de l’IVD postopératoire.</dc:description>
<dc:description xsi:type="unistra:Resume" xml:langue="fre">BACKGROUND : Right ventricular failure (RVF) after left ventricular assist device (LVAD) implantation is associated with significant mortality and morbidity. The objective of this study was to determine risk factors associated with the occurrence of RVF. METHODS : 68 patients was included. The primary endpoint was the occurrence of RVF. RESULTS : RVF after LVAD implantation was observed in 32 patients. In univariate analysis, an elevated SGOT (P=0.028) and a high preoperative vasoactive inotropic score (VIS) were significantly associated with an increased risk of RVF, whereas the implantation of LVAD through a thoracotomy approach was associated with a significant reduction in this risk (P=0.006). The multivariate analysis demonstrated that only the thoracotomy approach was significantly associated with decreased risk of RVF (odds ratio=0.33, 95% confidence interval=0.17-0.96; P=0.042). CONCLUSIONS : LVAD implantation by thoracotomy significantly reduced the risk of postoperative RVF.</dc:description>
