Une étude américaine a montré l’efficacité d’une affiche d’engagement public, basée sur la méthode du nudge, pour diminuer les prescriptions inutiles d’antibiotiques. Une étude contrôlée randomisée en simple aveugle a été menée auprès de médecins généralistes installés en Alsace. À l’automne 2017, une stratégie innovante, avec proposition d’une affiche, a été expérimentée dans le groupe nudge. Le groupe témoin a eu une intervention habituelle de l’Assurance Maladie avec remise d’un profil d’activité. Entre les hivers 2016-2017 et 2017-2018, le taux ajusté de consultations ayant débouché sur la prescription d’un antibiotique est passé de 15,5% à 13,4% (-2,1 points) pour le groupe témoin, et de 16,1% à 14,3% (-1,9 point) pour le groupe nudge, soit une différence d’évolution de 0,3 point (ICr95% [-2,0 ; 2,6]). L’affiche pourrait être un outil parmi d’autres pour soutenir l’utilisation raisonnée des antibiotiques. Summary : In France, consumption of antibiotics is increasing in primary care. An American study has shown the effectiveness of a public engagement through a poster-sized message, based on the nudging method, in reducing unnecessary antibiotic prescription. A randomized single-blind controlled trial was conducted among general practitioners established in Alsace. In the fall of 2017, an innovative strategy with a poster-mediated proposal was tested in the nudge group. The control group had a routine intervention of French National Health Insurance with the delivery of an activity profile. Between the 2016 and 2017 winters, the adjusted rate of consultations leading to prescription of an antibiotic decreased from 15.5% to 13.4% (- 2.1 points) for the control group, and from 16.1% to 14.3% (-1.9 point) for the nudge group, an evolution difference of 0.3 point (95% credible interval : -2.0 to 2.6).
