Intêret de l'échocardiographie transoesophagienne dans l'optimisation de la stratégie thérapeutique après un accident vasculaire cérébral

Maladies vasculaires, Introduction : la place de l’échocardiographie transœsophagienne (ETO) dans le bilan étiologique d’un accident vasculaire cérébral (AVC) reste controversée. Objectif : Le but de notre étude a été (i) d’évaluer l’incidence de la réalisation des ETO et des anomalies structurelles chez les patients ayant présenté un AVC, (ii) de préciser l’indice de sanctions thérapeutiques secondaires à la mise en évidence d’anomalies ETO et enfin (iii) de préciser le profil de tolérance et de sécurité de l’ETO dans l’AVC. Méthodologie : 990 patients consécutifs ont été admis pour AVC au CHU de Strasbourg de novembre 2017 à décembre 2018. Une ETO était « informative » lorsqu’elle révélait une anomalie structurelle (FOP, endocardite, thrombus etc.). Une ETO était « thérapeutique » lorsque son résultat impliquait une sanction thérapeutique (geste endo-vasculaire, introduction d’un traitement anticoagulant etc.). Résultats : 432 patients (43,6 % de la population totale) ont bénéficié d’une ETO, avec une ETO informative pour 224 patients (51,9 % des ETO) et thérapeutique pour 31 patients (7,2 % des ETO). Les étiologies d’une ETO informative étaient les suivantes, par ordre de fréquence : foramen ovale perméable (59,0 %), plaques d’athérome au niveau de l’aorte (52,0 %), ASIA (21,1%), contraste intra- OG (6,2 %), thrombus intra-cardiaque (3,9 %), contraste intra-AG (3,5 %), thrombus intra prothétique (1,8 %). La fermeture de FOP était la modification thérapeutique la plus fréquente (51,6 %). L’antécédent d’AOMI, l’antécédent d’IDM et le volume de l’oreillette gauche étaient les principaux facteurs prédictifs d’une ETO informative. En ce qui concerne l’ETO thérapeutique, l’âge, l’antécédent d’AVC ischémique et d’AIT, l’antécédent de FA/flutter, le traitement par AVK à l’admission, la topographie de l’AVC (sylvien superficiel et cérébelleux), le taux d’hémoglobine à l’admission ainsi que la survenue d’ACFA de novo étaient des facteurs prédictifs d’une ETO modifiant la prise en charge. En analyse multivariée, seul l’âge (OR : 0,948, 95 % IC : 0,923-0,974; p - 0.001), l’antécédent d’AVC ischémique (OR : 3,542, 95 % IC : 1,290-9,722; p = 0.01), d’AIT (OR : 7,830, 95 % IC : 2,214-27,689; p = 0.001), et la survenue d’un AVC sylvien superficiel (OR : 2,774, 95 % IC : 1,168-6,589; p = 0.021) étaient des facteurs prédictifs indépendant d’une ETO modifiant la prise en charge. Conclusion : l’ETO dans le bilan étiologique d’un AVC est majoritairement informative mais modifie la stratégie thérapeutique dans moins de 10% des cas, Introduction: The role of transoesophageal echocardiography (TEE) in the etiological assessment of a stroke remains controversial. Purpose: The purpose of our study was (i) to assess the impact of performing ETO and structural abnormalities in patients with stroke, (ii) to specify the index of therapeutic sanctions secondary to the evidence of ETO abnormalities and finally (iii) to clarify the safety and safety profile of ETO in stroke. Methodology: 990 consecutive patients were admitted for stroke to the Strasbourg University Hospital from November 2017 to December 2018. An ETO was "informative" when it revealed a structural anomaly (FOP, endocarditis, thrombus, etc.). An ETO was "therapeutic" when its result implied a therapeutic sanction (endovascular action, introduction of anticoagulant treatment, etc.). Results: 432 patients (43.6% of the total population) received an ETO, with an informative ETO for 224 patients (51.9% of the ETO) and therapeutic for 31 patients (7.2% of the ETO). The etiologies of an informative ETO were as follows, in order of frequency: patent foramen ovale (59.0%), atheroma plaques at the aortic level (52.0%), ASIA (21.1%) , intra-OG contrast (6.2%), intracardiac thrombus (3.9%), intra-AG contrast (3.5%), intra-prosthetic thrombus (1.8%). FOP closure was the most common therapeutic change (51.6%). The history of AOMI, the antecedent of IDM and the volume of the left atrium were the main predictors of an informative ETO. With respect to therapeutic ETO, age, history of ischemic stroke and TIA, history of FA / flutter, treatment with AVK at admission, topography of stroke (sylvian superficial and cerebellar), hemoglobin on admission, and onset of ACFA de novo were predictive factors for a treatment-modifying ETO. In multivariate analysis, only age (OR: 0.948, 95% CI: 0.923-0.974, p -0.001), history of ischemic stroke (OR: 3.542, 95% CI: 1.290-9.722, p = 0.01) , AIT (OR: 7.830, 95% CI: 2.244-27.689, p = 0.001), and the occurrence of a superficial sylvian stroke (OR: 2.774, 95% CI: 1.168-6.589, p = 0.021) were predictive factors independent of an ETO modifying the management. Conclusion: ETO in the etiological assessment of a stroke is mainly informative but modifies the therapeutic strategy in less than 10% of cases
