Création d'un score clinico-biologique d'exclusion de l'embolie pulmonaire. Étude rétrospective (mai 2017- avril 2018) sur 763 patients

Description : Médecine générale, INTRODUCTION : Aucun examen ne permet à lui seul d’exclure le diagnostic d’embolie pulmonaire, qui repose sur un faisceau d’arguments comprenant la probabilité clinique pré test, les D-dimères et l’imagerie pulmonaire. Le manque de spécificité des D-dimères conduit à réaliser de nombreuses imageries inutiles générant une irradiation des patients, un risque de réaction à l’injection de produit de contraste, un surcoût et une augmentation du temps de séjour aux urgences. Plusieurs scores d’exclusion ont été proposés mais la généralisation de leur utilisation n’est actuellement pas recommandée. L’objectif de notre étude était de tester rétrospectivement un nouveau score clinico-biologique d’exclusion de l’embolie pulmonaire. MATERIELS ET METHODES : Un total de 763 dossiers de patients consultant aux urgences de Strasbourg entre mai 2017 et avril 2018 pour une symptomatologie compatible avec une embolie pulmonaire ont été inclus pour tester ce score. RESULTATS : En utilisant un seuil de 2 points, la sensibilité du score était de 50,4 %, la spécificité de 80,1 % et sa VPN de 74,7% [IC 0,707 ; 0,783]. On aurait évité ainsi 529 scanners mais 134 patients ayant une EP auraient été non diagnostiqués (17,6%). Avec l’ajout de la variable « 3 critères avec astérisque ou plus » la sensibilité a augmenté à 72,5 %, la VPN à 80,3 % mais la spécificité a diminué à 61,1 %. L’analyse de la cohorte « D-dimères non interprétables » a retrouvé des résultats similaires à l’étude globale (Se 73,8 %, Sp 54,6 %, VPN 78,5 %). CONCLUSION : La VPN de 80,3 % signifie 20 % des patients ayant une embolie pulmonaire ne seraient pas diagnostiqués. Ces résultats ne permettent pas de recommander la généralisation de l’utilisation du score clinico-biologique avec le codage actuel. Toujours dans l’objectif d’améliorer la prise en charge diagnostique de l’embolie pulmonaire, une étude ultérieure, en révisant le codage du score pour améliorer sa performance, pourrait être envisagée., INTRODUCTION: No exam alone can exclude the diagnosis of pulmonary embolism, which is based on a set of arguments including the pre test probability, D-dimers and pulmonary imaging. Several exclusion scores have been proposed but the generalization of their use is currently not recommended. The objective of our study was to retrospectively test a new clinical-biological score for exclusion of pulmonary embolism. MATERIALS AND METHODS: A total of 763 files of patients consulting the Strasbourg emergency department between May 2017 and April 2018 for a symptomatology compatible with pulmonary embolism were included to test this score. RESULTS: Using a threshold of 2 points, the sensitivity of the score was 50.4%, the specificity was 80.1% and its NPV was 74.7% [CI 0.707; 0.783]. 529 scanners would have been avoided, but 134 patients with PE would have been undiagnosed (17.6%). With the addition of the variable "3 criteria with an asterisk or more" the sensitivity increased to 72.5%, the NPV to 80.3% but the specificity decreased to 61.1%. Analysis of the “non-interpretable D-dimers” cohort found similar results to the overall study (Se 73.8%, Sp 54.6%, NP 78.5%). CONCLUSION: The 80.3% NPV means 20% of patients with pulmonary embolism would be undiagnosed. These results do not make it possible to recommend the generalization of the use of the clinical-biological score with the current coding. Still with the aim of improving the diagnostic management of pulmonary embolism, a further study, by revising the coding of the score to improve its performance, could be considered.
