Consultations pour infection respiratoire haute en médecine générale : étude des pratiques de prescription de la corticothérapie et des éléments décisionnels des médecins généralistes alsaciens par une recherche observationnelle de terrain

Description : Médecine générale, Pour les infections respiratoires hautes, les corticoïdes oraux sont recommandés pour la laryngite, certaines formes sévères d’angines à EBV et les sinusites maxillaires hyperalgiques de l’adulte sous antibiothérapie. L’objectif principal était d’évaluer le taux de prescription des corticoïdes dans les IRH en médecine générale en Alsace au cours de la période hivernale chez les enfants et les adultes. Nous avons réalisé une étude observationnelle épidémiologique auprès de maîtres de stage universitaire. Au cours de cette étude, 21,1% des consultations observées concernaient une IRH parmi lesquelles 14,7% ont abouti à une prescription de corticoïdes per os. Il existait des facteurs associés de façon significative à la prescription de corticoïdes. Notre étude faisait ressortir un faible taux de prescription contrairement aux autres études. Une grande majorité des prescriptions étaient conformes aux recommandations. Des études plus précises sur les modalités de prescription doivent être réalisées., Introduction: High respiratory infections (HRI) are the first reason for consultation in general medicine practice. According to the literature, oral corticosteroids are recommended for laryngitis, some severe forms of angina with EBV and hyperalgic maxillary sinusitis of the adult on antibiotic therapy. The purposes of this study were to evaluate the prescribing practices of corticosteroid therapy for high respiratory infection consultations in general medicine practice and the decision-making elements of Alsatian general practitioners through observational field research. Materials and methods: We did an observational epidemiological study with university internship teachers which started in 2016. Eleven internal investigators were recruited. The data collection consisted of the filling of an "ECOGEN" grid for all the consultations observed and this grid was supplemented by a specific grid to collect more accurate data on the diagnosis and prescriptions made under the HRI. Results: In this study, 21.1% of the consultations observed involved HRI, of which 14.7% resulted in a prescription for corticosteroids. The four main significant factors that emerged for the prescription of oral corticosteroids were the associated antibiotic therapy, the type of HRI (laryngitis, maxillary sinusitis and rhinopharyngitis), certain characteristics of the clinical examination and other associated treatments. Eighty per cent of prescriptions were recommended or adapted. Discussion: Our study shows a low rate of prescription compared to other studies. A large majority of the requirements comply with the recommendations. More precise studies on the prescription modalities must be carried out.
