Intérêt d’un test de biologie moléculaire rapide couplant grippe et VRS dans la prise en charge des patients aux urgences et en réanimation

Description : Médecine générale, Durant l’hiver 2018-19, nous avons évalué le test moléculaire Cepheid dual® grippe/VRS réalisé 24h/24 au laboratoire de l’hôpital de Colmar, sur la prise en charge des patients aux urgences et en réanimation. Les chiffres concernant la grippe ont été comparés à ceux de l’épidémie 2017-18 obtenu par RT-PCR classique et test moléculaire rapide Alere-i® au lit du malade. Pour la grippe, 323 tests sont positifs, 188 adultes et 135 enfants. Chez l’adulte, le taux d’hospitalisation, le nombre d’antibiothérapie, le temps entre l’admission et la première dose d’OSELTAMIVIR sont significativement plus faibles dans le groupe Cepheid dual par rapport à la RT-PCR. Il n’y a pas de différence par rapport au test Alere-i®. Pour le VRS, 169 tests sont positifs, 41 adultes, 127 enfants. Le taux d’hospitalisation est de 70.7% (29/41) chez l’adulte et 76.4% (97/127) chez l’enfant. La durée moyenne d’hospitalisation est de 10,8 jours chez l’adulte. Une antibiothérapie est initiée chez 61.9% des adultes., During the winter of 2018-19, we evaluated the molecular Cepheid dual® influenza/VRS test used 24/24 in the laboratory of the Colmar hospital on the management of patients in emergency departement and intensive care. We compared this test to the 2017-18 flu epidemic results obtained by RT-PCR and Point-of-care Alere-i® molecular test. For the flu, 323 tests are positive, 188 adults and 135 children. In adults, the rate hospitalization, the number of antibiotics and the time between admission and first dose of OSELTAMIVIR are significantly lower in the group Cepheid dual compared to conventional RT-PCR. There is no difference by compared to the Alere-i® test. For the RSV, 169 tests are positive, 41 adults, 127 children. The hospitalization rate is 70.7% (29/41) in adults and 76.4% (97/127) in children. The average length of hospital stay is 10.8 days in adults. Antibiotic therapy is initiated in 61.9% of adults.
