Difficultés des médecins généralistes face au surpoids et à l’obésité. État des lieux des outils utilisés en pratique dans la prise en charge du surpoids et de l’obésité, à travers une étude qualitative bas-rhinoise

Description : Médecine générale, Résumé : l'obésité est considérée comme une pathologie chronique pouvant avoir des conséquences négatives sur la santé. Le médecin généraliste joue un rôle de premier recours dans la prise en charge de l'excès pondéral. Néanmoins, il exprime des difficultés face à cette pathologie complexe. Le but de cette étude est de réaliser un état des lieux des outils utilisés en pratique dans la prise en charge du surpoids et de l'obésité, afin de construire une plateforme de partage de données pour les médecins généralistes. Nous avons réalisé une étude qualitative en se basant sur des entretiens semi-dirigés auprès de quatorze professionnels de santé. L'étude s'est déroulée dans le Bas-Rhin, d'octobre 2017 à septembre 2018. Notre étude nous a permis de dégager différents aspects de la prise en charge de l'excès pondéral. L'initiation d'une prise en charge nécessite la reconnaissance du problème du poids par le patient et un certain degré de motivation pour entrer dans une démarche active. L'apprentissage de l'équilibre alimentaire par l'éducation thérapeutique est une alternative efficace aux régimes restrictifs. L'autonomie est rendue au patient, afin que celui-ci soit libre de faire ses associations de manière réfléchie. Le patient apprend à bouger son corps comme besoin physiologique afin de limiter sa sédentarité. Les troubles du comportements alimentaires doivent être identifiés et pris en charge par la thérapie comportementale ou la psychanalyse. La thérapie de soutien complète le suivi et permet de travailler sur le vécu et l'image du corps souvent négatifs, afin que le patient retrouve une harmonie de vie dans notre société. Le médecin généraliste peut déléguer certains aspects de la prise en charge à d'autres professionnels de santé afin que celle-ci soit la plus complète possible. Le patient en excès pondéral doit bénéficier d'une approche globale dans sa prise en charge, au niveau somatique, psychologique et social. Cette étude a permis l'élaboration d'un outil informatique de partage de données, morphosanté.fr, à l'usage des médecins généralistes afin de leur fournir des informations, des méthodes pratiques complémentaires et des pistes d'amélioration de la prise en charge adaptées à leurs patients. L'élaboration de ce site internet a été réalisée dans un second travail, Summary : Obesity is a chronic disease that can have serious health consequences. General practitioners play a central role in the management and treatment of obese and overweight patients. However, these often express their difficulties to deal with this complex pathology. The aim of this study is to undertake an inventory existing tools and methodologies used by general practitioners for the management and treatment of overweight patients, with the aim to build a repository platform of good practices to be used by general practitioners. To do so, we have conducted a qualitative study based on semi-structured interviews with fourteen health professionals. The study took place in Alsace from October 2017 to September 2018. Our study has allowed us to identify several key aspects of the management of obese and overweight patients. First, the effective management of this pathology requires the patient's recognition of the weight problem as well as a certain degree of motivation to actively address this problem. Importantly, teaching the patient to maintain a balanced diet through nutritional education, rather than the proposition of heavy dietary restrictions, appears to be an effective tool to address overweight problems. This gives the patient a greater flexibility and nutritional autonomy to the patient, allowing him to gain ownership of its nutrition in a more thoughtful way. In parallel, the patient learns to move his body as a physiological need in order to limit his sedentary lifestyle. Eating disorders must be identified and managed through behavioural therapy or psychoanalysis. Support therapy completes the follow-up and makes it possible to work on the often-negative experience and body image. The general practitioner may delegate certain aspects of care to other health professionals in order to ensure that the management and treatment of overweight and obese patients is as complete as possible. To be effective, the patient must benefit from a comprehensive approach, covering the somatic, psychological and social aspects of the treatment of overweight. The findings from this study have enabled the development of an online platform, morphosanté.fr, which will act as a tool for sharing good practices among general practitioners, providing them with information and practical methods to improve management and treatment of overweight and obese patients. The development of this online tool has been carried out in a second thesis.
